HPFT’s Specialist Learning Disability Services are looking for a highly motivated, compassionate, and experienced Senior Community learning disabilities nurse  to work across Buckinghamshire leading the work of the nursing team.

The diverse and multi-disciplinary Specialist Learning Disability Services provide specialist care to adults with Learning Disabilities whose needs cannot be met by mainstream services alone. The successful candidate will be based at our Hub at  Midshires Business Park in Aylesbury or Anglo Office Park in High Wycombe on personal choice.

As our new Senior Community learning disabilities nurse , you will be welcomed into a safe and supportive staff team, who empower and encourage one another in being the best version of ourselves for the benefit of those we care for.

Under the direction of the Team Leader, you will provide comprehensive professional and effective leadership, as well as clinical advice and expertise. Working hours Monday to Friday 9am to 5pm.

Detailed job description and main responsibilities

In your new role you will:

  • Be responsible for supporting service users with a learning disability and health need in the community. 
  • Carry out assessment of individual needs, make recommendations of how to meet those needs, support the implementation of these recommendations and monitor, evaluate and review the input
  • Work in close liaison with Team Manager, Nurse Team Leader, and Clinical Leads in delivering effective evidence-based practice to adults with Learning Disabilities requiring mental health services
  • Take a lead in and share the responsibility for identifying and developing opportunities for Service Users with learning disabilities to be enabled to comment on local and Trust wide service operational and development.

Person specification

KNOWLEDGE, TRAINING AND EXPERIENCE

Essential criteria
  • RNLD / RNMH
  • ENB 997/998( Mentoring & Preceptorship) or equivalent
  • Post graduate qualification
  • Experience of working with adults with a learning disability & their carers in the community
Desirable criteria
  • RMN
  • NVQ Level D32/33 (Assessors qualification)
  • ECDL, Minimum modules 2 and 7( or a willingness to undertake)

Skills and Knowledge

Essential criteria
  • Knowledge of health related practice, policy and legal requirements e.g. Valuing People, National Service Framework for Mental Health, Green Light Tool Kit, Mental Health Act 1983, Mental Capacity Act 2005, Deprivation of Liberty Act 2005, Care Programme Approach, Practice Governance
  • Experience in management of junior staff and ensuring service priorities are met
  • Proven skills in clinical and managerial supervision
  • Experience & skills in management of stressful situations ensuring recognition and support to staff and service users
  • Knowledge of change management
  • To have knowledge, experience and continuing demonstrable interest in learning disability nurse development
  • Teaching and presentation skills
  • Audit as a part of performance review and improving clinical practice
  • Advanced knowledge & skills in Risk Assessment & Risk Management
  • Ability to lead on Nursing Assessment’s, Devise, Implement and Evaluate Care Plans

COMMUNICATION SKILLS

Essential criteria
  • Possess excellent communication skills * in a variety of mediums, i.e. spoken, written & electronic
  • Highly developed skills in use of non-verbal communication

ANALYTICAL SKILLS

Essential criteria
  • Experience of decision making in a crisis
  • Knowledge & skills of non adverse risk assessment & management planning
