Job overview

Leominster Community Hospital provides high quality patient care within a friendly team environment to a diverse client group within the GP Led 26 bedded inpatient facility taking step down patients from the acute hospital and step up patients from the local Community.

Flexibility is essential for the post and shifts will be worked on a rotational basis over 7 days a week to include bank holidays.

Main duties of the job

We work within a multidisciplinary team providing independence and patient centred care, implementing realistic individualised goals to proactive discharge planning.
