Community Staff Nurse - Ledbury

The Ledbury District Nursing team are currently recruiting for a band 5 Community Nurse to join our supportive and dedicated team, based at Ledbury . This will be for 22 hrs per week.

We are a supportive and hard-working team, providing care and support to patients in their own homes. Whilst we see the majority of our patients at home, we are also able to see patients in a clinic-based setting, at Ledbury Hospital. This is for patients that are non-housebound, but require the expertise of the district nursing team.

We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners.

You will be equipped for mobile working and will be provided with a laptop and smartphone, meaning that you will have access to patient data and are able to complete patient documentation and paperwork in real-time. All staff are expected to record their work electronically on EMIS, our electronic patient record system which links into the GP services to allow a seamless service provision. Training and on-going support will be given to support staff with EMIS and other electronic systems they will need to use.

After induction, the post holder will be required to work a mixture of shifts from 8am to 8pm - there are no night shifts.

You will be expected to use your own vehicle for travel, however you will be reimbursed for expenses occurred.

The focus of District Nursing is to support patients towards self-management, enabling them to continue to live as full and productive a life as possible, through education, equipment and or lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with the Hospice Teams and Specialist Palliative Care Nurses.

Providing a high standard of care to patients in their own home or clinic environment, this will include a number of clinical competencies:

• End of life care - syringe driver and symptom management.

• CVAD management

• Catheters - male, female and suprapubic

• Complex wound care

• Drain management

• Compression/Dopplers

• Bladder Scanning

• Holistic assessment of needs

• Skin Integrity

  • Nephrostomy care

There will be training provided via the Integrated Care Academy for the successful candidate.

  • Must hold the qualification of RGN, RSCN or RM and be registered with the Nursing and Midwifery Council.
