The key principles of Wye Valley Trust are to improve the health and well-being of the people we serve in Herefordshire and surrounding areas. We have an exciting opportunity for the right candidate to join our wonderful District Nursing Team. 

The Golden Valley District Nursing Team are a small and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes. Being able to work alone and able to travel in a very rural area is essential. 

Due to relocation of current staff we have two 30 hour posts available for the right candidates.

We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. 

We welcome applications from both experienced and newly qualified nurses. There is a fantastic preceptorship programme delivered by the Trust and you will be fully supported to complete all the clinical competencies via the Integrated Care Academy and be supported in practice to develop these skills by a clinical assessor.

Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.

Due to the nature of this role, the ability to meet the travel requirements of the post are essential.

Main duties of this role include, but are not limited to;

  • Wound care
    - Palliative care
    - Managing end of life symptoms and providing support for relatives/carers
    - Managing long term conditions in a community environment
    - CVAD and drain management
    - Chemotherapy disconnect
    - Holistic assessments
    - Post operative care following discharge
    - Wound assessments and management
    - Venous blood samples
