This role co-ordinates and develops high quality patient-focused rehabilitation and discharge.

Clinical experience desirable

Prif ddyletswyddau'r swydd

You will ensure continuity and quality of care by working in partnership with patients, carers and the multi-disciplinary teams in the planning and delivery of safe and timely rehabilitation and discharge whilst supporting the clinical team.

You will be a good communicator who is able to manage and prioritise own workload and can demonstrate sound IT ability

  • Foundation degree in Health and Social Care/equivalent knowledge or Vocational level 3 education or equivalent knowledge/skills AND Level 2 numeracy and literacy

  • Dementia Level 2/3
  • End of Life Level 2/3

Experience & Knowledge

  • Experience of working on an acute clinical area
  • Experience of multi-agency working
  • Multidisciplinary team working
  • Experience of working with IT systems
  • Ability to demonstrate the required behaviour in keeping with the Trust values
  • Ability to communicate written and verbal information in English, keeping legible and accurate records
  • Able to present information, written and verbally in a clear and logical manner
  • Ability to work without direct supervision and prioritise own case load

  • Awareness of local and national agenda within the speciality
  • Knowledge of Government publications/initiatives regarding delayed discharge and social care
  • Experience of patient discharge planning
  • Customer Service experience i.e. dealing with complaints
