All Age Continuing Care covers, Continuing Healthcare and NHS-funded Nursing Care as a
statutory duty of the Integrated Care Board for individuals with a ‘primary health need’, including Mental Health, Learning Disabilities, Physical Disability, and Fast Track.
All Age Continuing Care also has Integrated Care Board duties to children and young people (Children’s Continuing Care), joint packages of health and social care (where an individual is not eligible for Continuing Healthcare but specific needs have been identified through the Decision Support Tool that are beyond the powers of the local authority to meet on its own), personal health budgets, stand-alone health needs, s117 of the Mental Health Act and Specialist Rehabilitation (Acquired Brain Injury) .
All Age Continuing Care delivers the patient specific Learning Disabilities and Autism Programme Integrated Care Board responsibilities.
In some areas positions will be within an integrated role with duties across Integrated Commissiong or Local Authorities.

Detailed job description and main responsibilities

To work within the NHS All Age Continuing Care team to establish and maintain robust systems to implement current policy and guidance relating to the NHS Continuing Healthcare and All Age Continuing Care processes.
Manage a team of professional staff and administrators, providing clinical supervision and professional expertise around complex assessment or case management, appeals, NHS England Independent Review Panels and Court of Protection (as required) to ensure timely, safe and cost-effective delivery of services.
Manage clinical risk and ensure escalation of financial, quality, clinical or legal risk to seniors and the Hampshire and Isle of Wight Integrated Care Board (HIOWICB)
To contribute to the training and development of all staff (internal and external to the Hampshire and Isle of Wight Integrated Care Board (HIOWICB)) regarding policy, processes and guidance around NHS Continuing Healthcare, Funded Nursing Care, Fast Track, Children’s Complex Care and All Age Continuing Care.
Monitor and actively manage the clinical caseload and numbers of cases awaiting assessment, review or case management, supporting reporting mechanisms to ensure Hampshire and Isle of Wight Integrated Care Board (HIOWICB) is kept updated as part of performance assurance of NHS Continuing Healthcare.
Development and delivery of Cost Improvement Programmes.

Knowledge, Training and Experience

- Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE including AMHP’s and mental health social workers or significant relevant experience (LD/MH/LDAP teams only). ENB 998/Mentorship level 2/3. Experience of line managing staff. Educated to degree level.
- Demonstrates excellent knowledge of NHS Funded Continuing Health Care criteria – guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act etc.) Court of Protection/legal case management, responsible commissioner “who pays” guidance
- Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team)
- Demonstrates excellent knowledge of Clinical and Information Governance Frameworks.
- Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles)
- Good knowledge of Mental Capacity Act and relevant legislation.
- Good working knowledge of complaints procedure.
- Understanding of Confidentiality and Data Protection Act.
- National agendas and effect on health and social care practice e.g. National Health Service Frameworks.
- Participated in development of care standards, policies and services.
- Experience of working in an acute and/ or a community-based setting.
- Qualified Best Interest Assessor, for DOLS team
