 An exciting opportunity has arisen to be part of a vibrant and dynamic team working within an innovative and continually evolving service, Single Point of Clinical Access (SPCA).

The purpose of SPCA is to manage calls from health care professionals and service users with the common goal of avoiding acute hospital admissions, increasing the use of alternative community services and provision of out of hour’s dental self-help advice. SPCA actively re-directs requests and referrals on behalf of referrers, for access to multiple alternative services.

SPCA also manages countywide community bed capacity for Gloucestershire and as part of the wider Patient flow Team, responds in supporting flow in a dynamic and timely manner during times of system wide escalation, whilst also working very closely with system wide multidisciplinary teams to support other alternative pathways of care.

Main duties of the job

The successful applicant will need to demonstrate relevant clinical and specialist experience that will support triaging referred/self-referring patients over the telephone and managing calls from health care professionals and stakeholders.

Key to success will require you to support development of the service and to have an overview and understanding of daily capacity, demand and flow of the service as well as the organisation as a whole.

On a day to day basis, SPCA service delivery takes place in an often-pressurised environment and requires quick decision making and the ability to be able to flex to meet service demand. Excellent communication and assessment skills are of paramount importance and due to the nature of the work a knowledge/experience of reablement and community-based services would be advantageous.

The successful applicant will need the ability to work on site (SPCA office is based in Edward Jenner Court, Brockworth) and visit Gloucestershire Health and Care NHS Foundation Trust, Gloucestershire Hospitals NHS Foundation Trust, South Western Ambulance Service NHS Foundation Trust and Integrated Care Board services with face to face patient contact as required. There is a requirement to use display screen equipment and wear a telephone headset therefore you may be restricted to your personal workspace for the majority of the shift.

Qualifications

Essential criteria
  • Degree or equivalent experience/training/knowledge relevant to position
  • Registered professional NMC, HCPC etc
  • Knowledge of evidence based practice
  • Experience working in health and/or social care
  • Knowledge and understanding of current health and social care policy

  • Teaching and assessing qualification

Experience

Essential criteria
  • Experience in assessing and planning care for complex patients across both primary care and acute settings
  • Experience of multi-agency working
  • Knowledge of relevant codes of professional conduct and scope of practice (AHP, NMC, HCPC,CSP)
