Trosolwg o'r swydd

·         To work as an independent practitioner as part of the HIV Specialist Nursing Team to assess, advise, treat, monitor and support HIV patients within Gloucestershire.

·         To provide a high quality specialist nursing service to patients, relatives and carers who attend the HIV outpatient clinics in Gloucestershire, and provide a confidential and consistent high standard of care in accordance with the National HIV Care Pathway & BHIVA guidelines.

·         Work as part of a multidisciplinary team with other nursing colleagues, pharmacist and pharmacy technician, medical colleagues, administration and support workers.  To liaise with external agencies e.g. social care, HIV support agencies.

·         To undertake Sexual Health history taking, including triage and contact tracing and liaison with Health Advising team and other Sexual Health clinicians.

·         To undertake nurse-led HIV six monthly review consultations for patients with stable HIV which includes comprehensive medical and sexual health assessment.

·         Professionally lead the nursing team and team in the absence of the HIV Lead Nurse.

·         To support the HIV Lead Nurse with the development of HIV care pathways and protocols and to implement new national initiatives.

·         To provide the full range of specialist clinical nursing skills within the HIV service.  This may include evenings and weekends as the service develops to meet the evolving needs of HIV care.

·         To provide a seamless high quality service from referral through to assessment, diagnosis, treatment and review, referring to other more experienced members of the multidisciplinary HIV team according to patient need.

·         To undertake nurse-led HIV consultations for patients with stable HIV and is able to use professional judgement to seek advice or refer to medical colleagues.

·         Administer or issue prescribed medication as appropriate following NMC professional guidelines, working from PGDs (patient group directives) or PSD (person specific directives) as appropriate.

·         To review and interpret complex clinical blood results, triaging for consultant review within the multidisciplinary team working model.

·         To use professional judgement to seek advice or refer to medical colleagues.

·         Use professional judgement to intercede and act as advocate for patients to ensure a patient focussed approach to the delivery of care. Support and enable patients are carers to make informed decisions relating to their treatment and management. 

·         Maintain a good understanding and implementation of clinical escalation procedures as required.

·         Required to provide the confidential and comprehensive HIV standards of care as per the National HIV Care Pathway and BHIVA Guidelines. Provide Sexual Health care including contraception within the HIV clinic service.

·         Manage and communicate complex, highly sensitive, confidential information to patients and other agencies e.g. safeguarding, child protection, sexual assault, termination of pregnancy, learning disabilities and other additional health and social care needs. Communication requiring empathy, persuasion and reassurance in accordance with their health and social care needs; to identify and adapt communication to individuals who may have special needs, language or communication barriers, or be hostile and antagonistic

·         Provide health promotion and education relevant to patient needs.

·         Promote sexual health / contraception services by liaison and joint training with other departments / HIV charity Eddystone Trust to improve utilisation of these services.

·         To become competent in and use or be willing to undertake non-medical prescribing training to prescribe drugs for the HIV patients, including anti-retroviral drugs.
