Are you looking to progress your career? An excellent opportunity has arisen for a Mental Health Nurse with mental health experience in the Assertive Outreach team.
We are looking for an enthusiastic and motivated individual to join our multi-disciplinary team. Applicants should have an interest in working with individuals who have severe and enduring mental health problems and are difficult to engage. The specific purpose is to deliver care in a collaborative manner that reflects the principles of AOT working, particularly aspects relating to recovery and social inclusion.
This is a full-time post. At band 6.
The Gloucester and Forest of Dean Assertive Outreach Team provides comprehensive mental health and social care to working age adults in the Gloucester locality and the Forest of Dean.
The team provides a planned needs lead 8am-8pm (core hours 9-5) seven days a week service (reduced hours at weekend). The hours are 37.5 per week with some evenings and weekend shifts (on a rotational shift system). ‘Flexible’ working arrangements will be considered. Any request will be set against genuine operational requirements and limitations and it is not obligatory for a manager to accede to the request.

This is an exciting opportunity for a community mental health nurse who has some experience of working with adults of Working Age and caring for individuals with Enduring Mental Health problems. The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.

The post holder will be a senior member of the team, working with service users across the Strategic Service Units who have a common and severe mental health. The purpose of this role is to:
- Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs
- Optimise the service users ability to remain in the community and / or be cared for in the least restrictive environment in line with the principles or recovering and social inclusion and the 10 essential capabilities
- Support clinical governance initiatives at ward / hospital level e.g. Essence of Care Clinical Audit
