This new post is an exciting opportunity for an experienced Advanced Practitioner to join our Directorate.  We have an increasing number of Qualified and in Training Advanced Practitioners in both our inpatient beds and our urgent care service (Rapid Response) and we now seek an operationally focused leader to work with the Service Director and Medical Leadership team to ensure we continue to develop our model of care with assurance and good governance. 

The successful post holder will need to be experienced and creative; agile in thought and organised in their planning.  You will need to help create the environment for growth of these colleagues, ensure they are anchored into well supervised development pathways.  Wider than that you will be working with Trust and system partners to develop the advanced practice agenda and ensure that service delivery is optimised by the  recent inclusion of these roles.

This role brings the post holder an opportunity to work with service redesign and  transformation as well as ensuring the day to day provision of excellent care.  You will be required to work clinically for a proportion of the hours and retain all pillars of your practice as a core component of the role. 

Main duties of the job

Carry out clinical functions innate to the role, including clerking, medical planning, medicines reconciliation, prescribing/deprescribing, regular reviews, supporting rehabilitation, discharge planning and care in the last days of life. 
 Working with the learning and development team, devise and deliver education programmes for tAPs and APs within the organisation
 Work with advanced practitioners within the system and regionally to develop best practice
 Act as a clinical role model for developing and delivering care that is responsive to changing requirements.
 To constructively challenge others and demonstrate receptiveness to challenge to promote a culture of psychological safety, learning and reflection.
 Lead and participate actively in research and development initiatives to advance the provision of services for patients. Supervise and co-ordinate the work of other APs and TAPs in this space. 
 Facilitate the dissemination of research findings to influence change
 To lead/supervise Advanced Practice activities related to audit and quality improvement (QI), participating in updating and implementing policies, protocols and guidelines, in line with national guidance and contemporary evidence.

 Working with complex an undifferentiated clinical presentation and at times assessing and managing people exhibiting challenging behaviours and mental health needs.

 Working with agility to ensure the directorate is able to govern its Advanced Practitioners and trainees appropriately
 Working through influence, negotiation and collaboration with others to achieve impact, outcome and success

Person specification

Essential

Essential criteria
  • Registered Nurse/ AHP – NMC / HCPC Registration
  • MSc In Advanced Clinical Practice
  • Independent Non-medical prescribing
  • Educational Supervisor/mentorship qualification / Teaching and Assessing qualification Or relevant experience as a practice assessor / Practice Educator and / or Practice Supervisor
  • Leadership and Management qualification gained as part of the Masters level programme of study or equivalent experience gained in a senior leadership position
  • Post Masters development as part of CPD
  • Experience of clinical working within the frailty speciality
  • Evidence of leading change across at organisational level
  • Demonstrable knowledge and application of Clinical Governance processes
  • Highly specialist advanced clinical skills and knowledge
  • Currently fulfilling the role of educational supervisor
