 A challenging and rewarding opportunity for a Band 4 Heart Failure Educator has arisen within the Gloucestershire Countywide Heart Failure Service.

Main duties of the job

Based at Southgate Moorings, Gloucester. You will work as part of our Community based Heart Failure Team, delivering education to heart failure patients and their families, to empower them to manage this long term condition. To deliver heart failure education to fellow colleagues, to aid them in managing this long term condition. We are looking for a highly motivated, enthusiastic individual, who is confident in delivering education and can demonstrate a knowledge of heart failure management, to complement our dynamic team. Although teaching can be given. Knowledge of Primary Care working is desirable. You will need to be able to work independently and have excellent communication skills.

You will be supported in this role by the Clinical Nurse Specialists and GPs with a Specialist Interest in Heart Failure. Regular peer reviews are provided to support your practice and development in the role.

Detailed job description and main responsibilities

You will be required to provide education either in a group setting, or on an  individual basis within a patient’s own home, and manage the e learn platform. Providing education on healthy lifestyles and using evidence based management strategies to seek the best outcomes in patients living with heart failure. In line with GHC practice the service uses electronic patient records and Information Technology skills are essential. Your role will involve seeing patients in-group settings, at home and in clinic. 

 The successful applicant must hold a full UK driving licence and have access to a car for work purposes. This is a countywide role.

Person specification

Essential / Desirable

Essential criteria
  • NVQ Level 3 in health and social care management or similar qualification, or relevant experience
  • GCSEs English & Maths Grade C or above
  • European Computer Driving Licence (ECDL)
  • Experience of working in a health and social care environment role including exposure to patients with heart failure
  • Experience of working in the community setting
Desirable criteria
  • Health Coaching Qualification or willing to undertake
  • Experience or knowledge of health coaching techniques and how they can be used to facilitate positive behaviour change and motivation for the individual’s health and wellbeing

Essential/Desirable

Essential criteria
  • Have an understanding of the needs of people with chronic health problems, particularly in relation to promoting their independence.
  • Ability to work effectively alongside different service providers/patients/carers, supporting individuals to maximise self-management
Desirable criteria
  • Working knowledge of Microsoft Office packages e.g. System one, Word, Excel, PowerPoint, Publisher and Outlook
