If so this may be the role for you. Our Home First & Reablement service supports people return home after a stay in hospital. This helps them continue their recovery in their own surroundings – regaining their ability to live as independently as possible.
Our service runs 24/7 and all our Rehabilitation Assistant roles offer core hours of 07.00-22.00. Full-time roles operate on a 4-days on, 4-days off schedule and part-time roles consist of 2-days on, 6-days off.

Senior Rehabilitation Assistants work with our Occupational Therapists, Physiotherapists and Reablement Coordinators to undertake assessments and provide review to ensure people are receiving the right level of input and / or re working on their outcome based goals.
Senior Rehabilitation Assistants work as part of a close team undertaking activities according to people's individual goal plans, encouraging them to do as much as possible for themselves, to live well and safely in their own homes. They also work with the Reablement coordinators to support and maintain quality standards of work delivered by the Rehabilitation Assistants.
Your role will involve completing assessments, determining risks and how to manage them, communicating and recording your feedback to others involved in people's rehabilitation.
You will need to be able to drive around the locality to work with people in their homes. Team working and communication skills are essential. You will be confident with handling IT systems to update written notes and electronic records.
The service runs between 7am and 10pm, 365 days a year. There are pay enhancements for unsociable hours, as stipulated by Agenda for Change. For more information please see the Additional Information link to the shift patterns and search for NHS Unsocial Hours Payments for more information.

This will predominately involve:
- Within a shift, working in a number of persons’ own homes often with no other professional staff present, assessing and implementing risk mitigations as needed. As well as providing appropriate interventions which may involve practical, personal and intimate care.
- Communicating both verbally and in writing about a person’s progress in case notes, to members of the Reablement Team and others.
- Providing support to Rehabilitation Assistants with their ongoing work with people
in their own homes.
