Job overview

Working Well is commissioned by Essex County Council and sits within the Essex Wellbeing Service.  This fast paced, innovative programme that supports the national and local agenda around workplace health, wellbeing and inequalities, ensuring that the Essex workforce is healthy and productive. 

 

The role involves delivering a range of health initiatives as part of an outreach programme across Essex, including:  Stop Smoking Support and training, NHS Health Checks and lifestyle management.

Detailed job description and main responsibilities

Apply behaviour change techniques to support routine individual lifestyle changes, e.g. goals setting

 

· Provide evidence based information about a range of health issues such as smoking, alcohol, mental health, healthy eating and physical activity

 

· Social prescribe to other service that will enhance and sustain the outcome of the interventions.

 

· To work autonomously on non complex cases and in partnership with the Workplace Health Outreach Coordinator for complex cases

 

· To support asset mapping within the community

· To manage caseloads efficiently

· To undertake basic clinical interventions: blood pressure, BMI

· Provide Smoking Support and training for workplaces

· Contribute to and work as works as part of a team.

· To follow up client during and after completion of their lifestyle change intervention and discuss further steps.

 

· Undertake any other delegated work

· Any other function commensurate with competencies and job role

Qualifications and Education

Essential criteria
  • NVQ Level 3
Desirable criteria
  • Level 2 Health Trainer Qualification - Understanding Health Improvement (Royal Institute of Public Health)

Work Related Knowledge

Essential criteria
  • Health and social care or Equivalent training and experience in health care setting
  • Level 2 Stop Smoking Cessation
  • NHS Health Check training or willingness to learn

Personal Circumstances

Essential criteria
  • Car owner – must be able to travel to various areas of Essex as part of the job role. Be willing to drive a Provide CIC outreach vehicle
  • Flexible working – which will include evenings and weekends Willingness to work throughout all areas covered within Esse

Skills and Apptitude

Essential criteria
  • Experience of using a range of IT packages including Microsoft Office and Systmone
  • Knowledge and expertise to apply of behaviour change methods
  • Well organised and ability to prioritise workload and work unsupervised
Desirable criteria
  • Willingness to undertake further training as appropriate
