Job overview

We are seeking an enthusiastic Community Matron to join our Uttlesford South Integrated Care team working as a member of the South Uttlesford Integrated Neighbourhood  Team delivering care to patients closer to home, preventing unnecessary conveyance and hospital admission, and reducing unnecessary calls to the ambulance service, promoting the ‘Home First’ philosophy for the population of South Uttlesford.

Your role as part of the team will be integral in supporting the local population and the wider West Essex and Herts Integrated Care System.

The post holder will work within the Integrated Community Care Team to proactively manage, co-ordinate and support those people with multiple complex health conditions.

Candidates should be self-motivated with strong organisational skills and able to make complex clinical decisions autonomously as required. As a senior clinician you will use your advanced clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce likelihood of unnecessary admissions to hospital. You will be required to complete holistic, advanced assessments, giving appropriate interventions as required and have knowledge of community services to plan the on-going care of patients on the pathway.

Main duties of the job

This is an integral part of the West Essex Out of Hospital Strategy designed to ensure that patients receive appropriate  safe care and treatment as close to home as possible.  

Working autonomously as an advanced practitioner, the community matron will take a lead role, working within a designated Integrated Neighbourhood Team across boundaries to co-ordinate and ensure proactive care is delivered, responding to the early symptoms of disease exacerbation, processes, and current evidence and practice standards.

The post holder will deliver appropriate interventions and co-ordinate packages of care to a ‘High Risk’ population with complex needs and long term conditions in the community, with the aim of preventing inappropriate hospital admission and supporting safe early discharge from hospital.

 The post holder will also be responsible for the decisions about care and service provision for individual patient pathways across the primary and secondary care interface.

Detailed job description and main responsibilities

The community matron will work as part of the multidisciplinary Integrated Neighbourhood Team supporting a PCN to establish a whole systems approach to managing health and social care needs (where appropriate) for individuals with complex long term needs. In addition, the post holder will work closely with general practitioners the Urgent Response team, specialist community nursing and therapy teams, medicines optimisation,  adult social care colleagues, the voluntary sector and across the primary and secondary care interface to ensure that care is co-ordinated effectively for their patients and/or carers.
