REACT brings health, social care and voluntary sector (British Red Cross, Suffolk Family Carers and Citizens Advice Bureau) services together to provide a single coordinated care response that is underpinned by care in the community with early intervention, crisis management, reablement and rehabilitation.

An exciting opportunity has arisen for a middle grade doctor to join our award winning REACT (Reactive, Emergency, Assessment, Community Team) service. Based in the lovely county of Suffolk, this is a key role working within our virtual frailty ward and the 2-hour urgent community response across Ipswich & East Suffolk.

Main duties of the job

The virtual frailty ward seeks to manage individuals living with frailty who have an acute medical condition that would usually necessitate an admission to the hospital but with the support provided by the virtual frailty ward team, the individual will be supported to remain in their own home receiving a high level of clinical care, diagnostic and therapeutic interventions to manage the acute episode.

The urgent community response within REACT provides a two-hour response and subsequent   support to people who are experiencing a health or social care crisis that puts the individual at risk of not being safely managed within their own home with an increased risk of a hospital admission. This service enables adults with complex needs to access support from a wide range of health, voluntary and social care practitioners, supporting the individual’s independence if possible.

We are building a new elective orthopaedic centre for ESNEFT at Colchester Hospital. It will be one of the biggest centres in the country and will ensure we can retain complex and low volume cases locally for the people we serve.

We are also now building a £7million specialist interventional radiology and cardiac angiography unit at Colchester.

At Ipswich we are investing £35million in a new Emergency Department and co-located Urgent Treatment Centre, with additional diagnostic imaging kit (MRI and CT). There’s a £5.3million molecular diagnostic laboratory soon to open at Ipswich, and works begins soon for a new Breast Care Centre and a Children’s Unit.

Detailed job description and main responsibilities

The post holder will be responsible for providing the medical review of patients within their own home environment, who are on the REACT and virtual frailty ward caseload. This will include the planning and management of complete episodes of care, initiation and management of therapeutic diagnostics and interventions, working in partnership with the multidisciplinary team, and delegating and referring as appropriate to optimise health outcomes and resource use. The REACT caseload is discussed on the daily morning handover and the virtual ward has a twice weekly MDT. Senior medical cover is provided by Dr Lim, clinical lead for REACT, and Dr Dan Coates. There is also advice available from consultant covering the frailty assessment base Monday-Friday 9-5pm.

Initially this post will be Monday to Friday 8.30am-4.30pm with appropriate breaks but as the virtual frailty ward continues to develop then the post holder will be asked to participate in a rota that provides 7 day a week cover alongside the other locally employed doctor post and the two tACP posts. There will be opportunities to attend the frailty assessment base and falls clinic as a learning opportunity.

  • MBBS or equivalent
  • MRCP part 1
  • Some experience at CMT /VTS level of acute geriatric and general medicine
  • Some NHS experience
  • Evidence of working in a multidisciplinary team
  • Ability to organise and prioritise competing demands
  • Flexibility to respond to changing service needs
  • Valid driving license
