An exciting opportunity has arisen for permanent Band 5 Community nurses working within the South Rural Integrated Neighbourhood Team. You will be passionate, committed, innovative, enthusiastic, and highly motivated to delivering high quality care within a patient's home. 

The South Rural INT is based at Hadleigh Health Centre and East Bergholt, providing an opportunity to work within a knowledgeable experienced team and wider MDT.  You will be required to work across the South Rural INT and the wider community services to meet service requirements. The team consists of district & community nurses, assistant practitioners, generic workers, occupational therapists, physiotherapists, and social care workers. The team is led by a lead nurse and overall managed by an INT manager, supported by an administrator and care co-ordinator.

The team is proud of their holistic care approach and work shifts covering the operational hours of 08:00 to 18:00 over 7 days.

The team continues to change to support developments in the NHS which includes close working relationships with the local statutory, non-statutory and voluntary services. You will be expected to work autonomously and on behalf of the patients served in the local area. You should therefore be flexible, motivated, and willing to work within a changing environment.

Your duties would include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multi-disciplinary team meetings, PICC line care, male, female suprapubic catheterisation, ordering equipment, support and mentoring of students and involvement in link roles such as tissue viability.

We offer a variety of training to up skill and maintain your clinical skills.

Please note that Beacon House is a temporary location. This is likely to remain the teams base location for the next few years while building work is completed at the Hadleigh Health Centre. Once complete this role will be based at Hadleigh Health Centre.

The team will provide support through in-service training and professional forums, mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care.

Due to be a community post applicants must hold a UK driving licence and have access to transport from the start of their employment.
