Therapy Associate Practitioner - Stroke Rehab

An exciting opportunity has arisen for a band 4 Stroke Rehabilitation Therapy Associate Practitioner (TAP) to support our well-established Early supported discharge (ESD) service. With the publication of the new Integrated community stroke service (ICSS) model we are looking to deliver transformation and strategic changes in community stroke rehabilitation and life after stroke services that can influence the wider pathway and outcomes in mid & south Essex. Successful candidates will have previous experience working in stroke in any of the following areas: Psychology, Physiotherapy, Occupational Therapy, Speech & Language, or Health Care Assistant. They will also be passionate and motivated about enhancing the life of stroke survivors.
Full training and competency programme will be provided by a Psychologist, Physiotherapist, Occupational Therapist, Speech & Language Therapist and Nurses. and you will be supported by registered staff in the team to enhance your knowledge in this field and your ability to support stroke survivors.
On completion of competency programme, successful candidates will be able to manage their own non-complex caseload.
This is initially a 12 month project, which we are in the process of bidding for permanent funding for. This is not guaranteed and so the post is fixed-term at present

- Work within a multidisciplinary framework, undertaking a key-working role for non-complex patients as triaged by registered staff.
- Liaise closely with medical, nursing, allied health professionals, social care, acute and voluntary sector staff to promote greater understanding of the service. Support Staff in the delivery of therapeutic programmes on an individual basis or as a group. This will involve work from a variety of different disciplines with the post holder needing to prioritise and negotiate to ensure a manageable caseload is achieved in a timely manner.
- Implement, evaluate, and modify intervention, as directed.
- Be responsible for a delegated non-complex caseload following assessment by the registered staff, carrying out interventions, including goal setting, outcomes, risk assessment and discharge. However for complex patients follow the treatment plan and feedback on progress to registered staff and highlight need for re-assessment when needed.
- Monitor progress, taking account of the environment and adjust own clinical interventions accordingly
- Undertake risk assessments as part of everyday clinical practice, complete relevant documentation and seek guidance as necessary.
