Occupational Therapist

·       To perform advanced Occupational Therapy assessments of patients with diverse presentations and complex physical and psychological conditions. Use advanced clinical reasoning skills to develop and deliver individualised treatment programmes. 

To evaluate patient progress, reflect, analyse and reassess intervention and alter treatment programmes, as appropriate. Clearly communicating any changes in care plan or treatment approach to other team members to ensure consistency of approach and enhance recovery.

·       To maintain up to date and timely clinical records in accordance with service and professional standards and to access and update patient information as required. 

·       To hold clinical responsibility for a designated caseload, seeing patients in the most appropriate community setting.

·       To provide leadership and training for junior members of staff and support workers within the team and to supervise occupational therapy students on placement.

To work as an autonomous practitioner within the multidisciplinary team.

To undertake relevant competency training, specific to the service area and appropriate to the role banding. This may involve medicines administration and basic wound care.

 

 

 

 

 

Prif ddyletswyddau'r swydd

You will work as part of  multi disciplinary team, alongside physiotherapists, nurses, support workers,  associate practitioners and consultants.

You will be responsible for the assessment of patients allocated to you, and for the provision of their treatment plan and subsequent interventions, You will be required to complete joint visit with other members of the team. To be able to feedback to the cordintors in the serivce a clear plan for each patient. 

You will need to complete on ward referrals to other services, and ensure and make recommendations for the provision of equipment and or adaptations if deemed appropriate. 

You will also be involved in the assessment of patients with cognitive impairment, and dementia and will expected following appropriate training to be able to take patients observations, and complete clear concise and appropriate records.

You assessments will take place in a number of different settings but predominantly in the patients own home.

Qualifications and Education

  • Degree/ Diploma in Occupational Therapy
  • Health and Care Professions Registered
  • Portfolio evidence of post graduate training across a range of specialised clinical areas.

Work Related Knowledge and Experience

  • Broad range of clinical experience in a variety of settings including Community, Inpatient, Neurology, Orthopaedics and Care of the Elderly, of which some should have been within the NHS setting
  • Experience of managing own caseload including prioritising and delegating work in response to changing demands.
  • Demonstrates sound clinical reasoning skills.
  • Evidence of effective multidisciplinary team working
  • Supervision of junior staff, support workers and students.
  • Experience of prescribing a range of a range of specialist Occupational Therapy equipment to meet patient needs.
  • An understanding of national health care policy / national guidance relevant to this field.
  • Sound understanding of clinical governance / audit

  • Experience of working in an Admission Avoidance setting.

  • Ability to undertake the assessment of capacity, gaining valid informed consent and able to work within a legal frame with those who lack capacity to consent to treatment.
  • Competent moving and handling skills to enable accurate assessment of ability, to enable effective treatment and to optimise treatment outcomes.
  • Ability to work autonomously with support of formal and informal supervision.
  • Ability to work effectively under pressure
  • Ability to empower and motivate other, including patients and team members.
  • Demonstrate empathy with patients, carers and families often where barriers to understanding exist ensuring that effective communication is achieved to persuade and motivate participation in therapeutic activities
  • Able to communicate complex condition-related information to patients, carers families and multidisciplinary team members / other professionals, in written and verbal form
  • Negotiate with carers/patients/others around individual case management
  • Effective 1:1 and group teaching skills.
  • Able to delegate effectively and coordinate support worker’s workload.
  • High standard of written English and verbal communication skills.
  • Competent I.T. skills with Knowledge of Microsoft Outlook Packages
