Job overview

Do you aspire to provide holistic, patient centred care? Apply for our fixed term Community Nurse vacancy in Scarborough where you will provide assessment and care to patients in their own place of residence.  Because our service also provides nursing specialisms there is comprehensive support and advice available to you.

You will have a planned induction which includes a training programme delivered by our specialist nurses & for newly qualified staff we have a preceptorship programme. We have a clear competency framework so you know what is expected within your role & can plan development of your competencies with support from your line manager.

As a community nurse you will work closely with the wider MDT within our own services and other agencies including the acute hospitals, Hospice, General Practice, Social Services & the voluntary sector.

We are a 24/7 service so the role involves shift work via roster.

Working in the community requires a driving licence and access to your own vehicle. It is essential to be comfortable with lone working (policies and processes are in place to support lone working).

Main duties of the job

The main duties of the role include - 

  • Holistic patient assessment, formulation and reviewing of care plans
  • Holding a non-complex patient caseload
  • Management of pressure ulcers & wounds
  • Palliative & end of life care
  • Bladder & bowel care including catheterisation and continence assessments
  • Medicines Management
  • Injections (sub cut & IM)
  • Phlebotomy
  • Provision of equipment to support care
  • Safeguarding
  • Participation in daily safety huddles & handovers
  • Line management and supervision of junior staff
  • Practice supervision for nursing students
  • Continual learning and professional development

Qualifications

Essential criteria
  • Degree/diploma in adult nursing or in final year of training
  • Have current NMC registration or student registration if in final year of training
Desirable criteria
  • Up to date training & registration as a mentor
  • Evidence of relevant CPD – could include completion of short courses or post-graduate study
  • Completion of Nursing preceptorship

Knowledge & Experience

Essential criteria
  • Knowledge of the specific service area, team and role evidenced within the supporting information of application to vacancy.
  • Experience of Community Nursing in prior employment or student placement.
Desirable criteria
  • Experience of managing a caseload of patients including triage and prioritisation of referrals and daily work
  • Experience of mentoring pre-registration students

Skills and Competencies

Essential criteria
  • Evidence of how you will efficiently transport self and equipment to/from multiple venues over a large geographical area during each shift in the UK.
  • Adequate IT skills required to utilise electronic patient records, electronic communications, clinical stock ordering system and Microsoft Office applications.
Desirable criteria
  • Experience of use of SystmOne
