Job overview

We are looking for a highly motivated, confident Community Access Practitioner who has a strong interest in prison work.

The post will involve working with adult males within HMP Full Sutton or Millsike who present with risky behaviours and display signs of mental health difficulties.

The post holder will be working within our growing group of forensic services which is closely aligned  to our in-patient services and wider Humber & North Yorkshire Specialist Provider Collaborative.  Full and part time hours will be considered.

This is a great opportunity to be involved with new partnerships within the prisons; including Tees Esk and Wear Valley  NHS Foundation Trust as the lead provider delivering the mental health element alongside Rethink Mental Illness.

 

Main duties of the job

Community Access Practitioner will oversee prisoner pathways from reception into the prison estate through to release.   The role is responsible for forging working alliances with local inpatient and community partners to develop effective reintegration pathways for patients.   

The Community Access Practitioner will develop strong therapeutic working relationships with service users, offering consistent engagement from initial referral to an identified Mental Health/LD provision.

The Community Access Practitioner will develop robust working relationships with local and partner agencies and services across the statutory, voluntary, and independent sectors i.e., MAPPA, Private Providers and Local Authority Departments.

The post holder will deliver high quality care to a caseload of complex mental health and learning disability offenders as part of a liaison and an in-reach provision. 

The post holder will also be working within our growing group of forensic services which is closely aligned  to our in-patient services and wider Humber & North Yorkshire Specialist Provider Collaborative.

Qualifications and Knowledge

Essential criteria
  • Registered practitioner with professional body.
  • Professional qualification relevant to practice area, with study to degree level
  • Educated to master's Level or be able to demonstrate equivalent knowledge or experiential learning/experience
  • Full understanding/application of relevant clinical practice/standards/audit within identified service area
  • Detailed understanding of relevant policy, legislation, drivers and their application to clinical/service/organisation e.g. Mental Capacity Act, Health & Safety, Infection Control
  • Evidence of leadership skills and experience which have contributed to effective service delivery and development
Desirable criteria
  • Member of organisational group/network, locally or nationally
  • Be qualified to deliver a m ode of therapeutic intervention with clinical relevance to the role and service user

Experience

Essential criteria
  • Evidence of sound post-registration professional practice
  • Evidence of continuing professional development relevant to the professional lead area to advanced level of post-graduate diploma or equivalent knowledge / experiential learning
  • Proven leadership experience
  • Experience of organisational and professional networking
  • Ability to work across organisational boundaries, developing and maintaining multi-professional and multi-agency partnerships
Desirable criteria
  • Advanced management / leadership training / qualification or equivalent experience
  • Proven experience of developing others through education, mentorship, coaching, teaching, assessing, presentations, publishing
