Job overview

Would you like to work as part of our Intermediate Care Therapy Team covering Hull and East Riding ? We are looking for someone who has experience in delivering rehabilitation, to join our friendly team of therapists.

The successful candidate will already have knowledge and experience in assessing and treating people who have physical rehabilitation needs. You will be supervised by registered Occupational Therapy and Physiotherapy staff, and will follow their treatment plans , and you will also have your own caseload of patients who you will assess and treat. 

Main duties of the job

The main focus of the service is to prevent hospital admissions and to facilitate hospital discharges, enabling people to stay at home safely, and to be able to manage their everyday activities . Your role will involve visiting patients in their own homes in the community, and in community bedded units, so you will need to be able to travel across the area effectively.

The successful postholder will be working with a considerable degree of autonomy, and will be able to think on their feet and problem solve , but will have support available from the wider team at all times.

Essential criteria
  • Level 3 Qualification in relevant subject (Sports & Fitness/ Leisure Activity/Exercise Science/Health & Social Care)
Desirable criteria
  • Foundation Degree in Health & Social Care

Experience

Essential criteria
  • Experience of multi-disciplinary working
  • Previous Experience as a therapy assistant or health care assistant
  • Experience of working with people with physical and / or mental health problems.
  • Experience of working in a care environment, undertaking a full range of clinical competencies and maintains a competency portfolio.
Desirable criteria
  • Experience of working within rehabilitation services
  • Experience of working within the community
  • Experience of working with adults with complex needs
  • Experience of team working
