A rare opportunity has arisen for a band 5 registered nurse to work within the Urgent Community Response (UCR) team as part of the Intermediate care service.

We are looking for a dedicated and passionate band 5 nurse to join the team. The ideal candidate will assess patients within the Hull and East Riding geographical area and also service users based in a bedded unit. The ideal candidate should have some community experience, however, full training and support will be provided.

The post holder will work autonomously and also as part of a team to provide safe, effective, efficient care for patients, relatives and carers. They will be responsible for the assessment, planning, implementation and evaluation of programmes of care under indirect supervision from senior staff.

  • To support senior and to supervise junior staff in the provision of competent, sensitive and safe nursing care within the Community Ward/Community Short Stay Unit
  • Take responsibility for running of the department during a shift in the absence of more senior staff
  •  Take responsibility for assessment, planning, implementation and evaluation of nursing care for a group of patients
