Job overview

CARER HEALTH TEAM CLINICIAN  -BAND 6

 37.5 hrs

Are you a registered nurse or allied health professional looking to join an award-winning and innovative community team? If so, the Carers Health Team is the place for you!

In 2020 the service was highly commended by the NICE Shared Learning Awards for  ‘Implementing Clinical Support for Informal Carers’,  and in 2019 The Carers Health Team won the NHS Parliamentary Care and Compassion Award for the South East.

We provide professional health care input for informal adult carers across West Sussex.

The role involves lone working, home visiting, GP clinics, health promotion and outreach work (which can sometimes be either at the weekend or in the evening) including giving presentations and attending multidisciplinary meetings.

This role involves  caseload management and integrated partnership working with the voluntary sector.

Main duties of the job

The purpose of this service is to support carers through health and wellbeing interventions so that the carer and cared-for person achieve the best possible quality of life; relief from symptoms, and flexible choices about their health management. This is an anticipatory, health promoting model of care for adult carers and their cared-for and includes the identification of new carers.

In order to meet the stated outcomes you will be undertaking:

  • An holistic and clinical assessment of health, social and environmental needs of carers living in the community and carry a caseload of carers (and sometimes their cared-for) to enable proactive choices and advanced care planning with regard to the management of the condition and caring situation.

Working for our organisation

Sussex Community NHS Foundation Trust (SCFT) is the largest community healthcare provider in Sussex.

  • Over 8,400 full time, part time and bank staff members
  • Medical, nursing and therapeutic care provided to over 10,000 children and adults a day
  • July 2018 Care Quality Commission (CQC) rating of ‘Good’ with ‘Outstanding’ features
  • Flexible working patterns offered across all services
  • Thriving BAME, Disability and Wellbeing, LGBT+ and Religion and Belief staff networks
  • Accredited Level 3 Disability Confident Leader

SCFT is an inclusive employer that promotes equality and challenges discrimination, and welcomes applications from people of all backgrounds. Applications from Black, Asian and Minority Ethnic (BAME) candidates, disabled candidates and LGBT+ candidates are particularly encouraged, to ensure the Trust’s workforce reflects the diverse communities it serves.

Detailed job description and main responsibilities

We are looking for someone who has a nursing or allied health professional background who can show evidence of community based post qualification experience of working with informal carers. You will need to have a good understanding of the role of the informal carer and be someone who can enable carers to understand the importance of their own health and wellbeing needs as well as those of the person they are caring for.

You will be expected to work closely with Community Nursing Teams; Proactive Care Multi-Disciplinary Teams; G.P.’s & Primary Care Teams and other health and social care professionals along with other charitable and voluntary services in order to ensure a seamless and equitable service for carers.

Applicants will need to demonstrate a willingness to work flexibly and co-operatively with colleagues within the Carers Health Team and wider multidisciplinary teams across the whole SCFT area.

As the team work over the whole of the SCFT area, staff must be willing to travel within this area.
