Clinical Support worker - Higher level - Community Frailty

Our extensive health services are provided by over 7,000 dedicated members of staff working from two acute hospitals in Hastings and Eastbourne, three community hospitals in Bexhill, Rye and Uckfield, over 100 community sites across East Sussex, and in people’s own homes. 


In 2020 the Care Quality Commission (CQC) rated us as ‘Good’ overall, and ‘Outstanding’ for being Caring and Effective. The Conquest Hospital in Hastings and our Community Services were rated ‘Outstanding’ and Eastbourne DGH was rated ‘Good’ Our two acute hospitals have Emergency Departments and provide 24 hour a day care, offering a comprehensive range of surgical, medical, outpatient and maternity services, supported by a full range of diagnostic and therapy services. 


In the summer of 2020, the Trust launched its ambitious ‘Building for our Future’ (BFF) programme. This once in a lifetime programme aims to redevelop and expand our hospitals at Eastbourne, Hastings and Bexhill, transforming the environment in which we provide care for generations to come.

At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.

In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

Clinical Support worker - Higher level  - Community Frailty Practitioner Service

The ESHT community Frailty Practitioner Service is looking to recruit a Clinical support worker  to provide support to the Frailty Practitioner service across Eastbourne, Hailsham, Seaford, Hastings and Rother areas.

The Frailty Practitioner service works with older adults in the community living with long-term conditions and frailty combined with admissions to hospital. The aim of the service is to optimise clients’ health & medical treatment whilst providing them with the opportunity for advance care planning.

You will be motivated & organised, with the ability to work alone and as part of a closely integrated team. You will have strong communication & teamworking skills, a knowledge of the care of older adults and a willingness to learn about Frailty.

 

Prif ddyletswyddau'r swydd

You will work alongside Clinical Support Workers and Frailty Practitioners (Nurses and Physiotherapists) to assist with clinical assessment and to measure & record clinical information. You will provide frail patients with health education, health monitoring and support. You will also be required to take part in team development initiatives.

The successful candidate will work across H&R & EHS community locations. The office base is in Eastbourne/Hastings. You must have use of your own car to travel independently whilst at work and to carry small items of clinical equipment as required.

  • Care Certificate

  • 5 GCSE grade 4 or C and above or equivalent
  • Evidence of mandatory and statutory essential training skills, for example, Basic Life Support, manual handling, infection control, fire etc
  • Education and training to NVQ level 3/Work based diploma or equivalent

Experience
