Health Visitor, Healthy Futures team

Are you a health visitor who has wants to make a real difference to vulnerable children by ensuring they are ready for school? Then this opportunity is for you to work in a new team in West Sussex.

We are looking for an enthusiastic, experienced and innovative health visitor to work within the Healthy Futures Team who work with the transient population of people who may be Asylum Seekers, or Refugees.

This exciting initiative involves targeted, assertive outreach to communities experiencing multiple barriers to healthcare and aims to reduce both health inequalities and poor health outcomes.

This one year fixed term contract role will allow you to focus on these vulnerable children, supporting their health, development and wellbeing to ensure they are ready for school. 

As a health visitor you will be expected to undertake family health needs assessments and child health surveillance in accordance with the Healthy Child Programme.

You will work in partnership with children and families  to provide high quality effective healthcare, to prevent ill health, promote positive health and reduce health inequalities. You will support the development of the service for Asylum Seekers and Refugees.

Main duties of the job

To support the Healthy Futures Team to identify and  implement programmes of care to meet individual and local needs, taking into account cultural diversity and ensuring prioritisation of work load as necessary.

To offer a targeted service to children following their last core healthy child programme contact until they start school, to promote school readiness.

To work with families in a variety of settings providing evidence based advice and support.

To undertake family health needs  assessments and child health surveillance in accordance with the Healthy Child Programme.

 

To provide relevant information to families, to assist informed choices about their own and their family's health.

To operate within the Sussex Safeguarding Children Board and Child Protection and Safeguarding Procedures. To maintain and update knowledge around safeguarding children and young people and ensure that practice complies with local and national policies and legislation.

To manage a caseload of service users with complex needs within the community, using a broad range of assessment and intervention skills.

To work with other service providers  spanning medical, nursing, social care and allied health professionals as well as third-sector organisations.

We are recruiting to 0.6 – 1.0 wte Band 7 specialist public health nurses (SCPHN) roles to work with some of our most vulnerable families in West Sussex. 

These are new roles within our service to provide specialist roles within the team to lead on delivering the Healthy Child Programme to our most vulnerable families.  The current focus will be on leading in the delivery of the service to our asylum seeking and refugee children and their families. The role will be based in Crawley but will cover all areas of west Sussex and will be an integral part of the  0-5 healthy child programme team service delivery, having a specialist team caseload whilst working alongside colleagues to ensure the best health outcomes for all our families in our communities.

This requires a SCPHN with experience of working with complex families showing an ability to deliver excellent care with a flexible and compassionate approach. You will have excellent communication skills and show the ability to work in partnership with other organisations, developing relationships and pathways to achieve the best outcomes for the families you work with.

You will have excellent digital skills, being able to ensure data is recorded accurately and appropriately in a way that supports reporting of activity and outcomes and will work with the Team Leads and Clinical Service Manager to develop the service to meet the needs of the families.

The role will involve working as part of the Healthy Child programme Team, based in Crawley delivering the Healthy Child Program through targeted outreach and providing a trauma-informed approach. Previous experience working with these disadvantaged client groups would be advantageous but training, including cultural awareness, will be offered.

 Experience with line managing, delivering or setting up community projects would be advantageous, as well as safeguarding experience but training and development opportunities will be offered.

Whilst this role is based in Crawley, the families you will be required to work with may live else where within West Sussex so the ability to travel within your working day is essential.
