Job overview

·                Participate as an active member of the healthcare team assisting with patient flow and safe discharge of patients as determined by the 9A West Ward Manager and Junior sister.

·                To implement and evaluate the discharge needs of patients and to refer for the provision of these care needs to appropriate members of the multidisciplinary team.  

Main duties of the job

·         Specific responsibilities will vary between clinical areas (e.g. inpatient/ward versus Outpatient Department).  These will be subject to training, local protocols/Trust policies and formal assessment of competency.

·        Manage the discharge of an identified group of patients to ensure flow is maintained.  To ensure adherence to the UHS standard for patients to access their beds, in line with national standards.

·         Participate in MDT and discharge planning meetings. Update and implement daily discharge plans for a group of patients ensuring timely, safe patient discharge.  Recognising situations that may be detrimental to the health and safe discharge and escalating concerns to registered healthcare practitioner.  Updating and liaising with L9A West ward manager and Ward Coordinators, especially in times of Trust capacity issues.

·         Provide support and guidance to the multidisciplinary team, patients, carers/relatives regarding discharge/transfer of care promoting independence through.

·         Liaise with Occupational Therapist, Physiotherapist, Social Services, internal multidisciplinary teams, community teams and other professionals or agencies involved in discharge planning to support continuity of discharge planning for patients as directed by the registered practitioner.

·         Proactively complete and send referrals for internal departments or external agencies.

Detailed job description and main responsibilities

To promote and ensure optimum use of discharge lounge and maintain effective two-way communication and safe handover.

·         Complete patient assessments, plan and implement the care – evaluating its effectiveness, and ensuring delivery of high standards of evidence-based care.

·         Provide high quality patient care, appropriate to patient need.  Organise, prioritise and adjust own workload in relation to patient care.

·         Use knowledge and skills to make judgements on effectiveness of programmes of care, contribute to assessment of discharge needs, and discuss changes that might affect improvements for the patient.

·         Work always in a way that minimises the risk of infection (to patients and self) and maintains health, safety, and security in line with established protocols (e.g. hand hygiene, use of Personal Protective Equipment).

·         Work always in a way that maintains patient dignity, privacy, and confidentiality.

·         Recognise and understand end of life care and implement this according to individualised need.

·         Follow written policies, protocols/operating and discharge procedures and patient treatment/care plans, ensuring provision of a safe and therapeutic environment.
