We are looking to welcome a dynamic and innovative Youth Worker within the Children and Young Person’s Diabetes service. This fantastic opportunity will expand the care and engagement we have with our young people aged 11-19 years old as they move through their adolescent to adult life path.
Our team provides specialist paediatric diabetes support, education and advice to children, young people and their families with diabetes. Previous experience in Paediatric diabetes care is desirable but not essential as in-house education will be provided.
The role is a 1 year fixed term post for 22.5 hours/week Monday- Friday (potential for weekend/evening work) – Flexible working hours acknowledging that youth work may involve tailoring working hours to the young people’s requirements.
The role involves providing consistent care to the patients & families on our caseload and may include both in and out of hospital engagement. You will work to manage change in a positive and effective way and show flexibility within the role requirements.
Post holders are required to organise their own travel arrangements to meet the needs of the service.

The post holder will play a vital role in the development and implementation of our transition service. To walk alongside our young people as they progress (transition) out of paediatric diabetes care and integrate into the young adult service. By supporting young people, we aim to understand and grow their strengths and skills, empower them to take action, and facilitate positive change relating to individual health needs.
The post holder will devise and develop ways in which peer support can be achieved. Be involved in supporting a range of age appropriate recreational/youth work activities. Provide outreach to young people struggling to engage with health services. Work to ensure the clinic environment is young person friendly and meets their needs.

As a Youth Worker you will use your experience of working with children and young people (11-19years) to:
- Support young people to understand and grow their strengths and skills, empowering them to take action, and facilitating positive change relating to their individual health needs (Diabetes and related challenges).
- Work with a range of colleagues to grow and enhance our engagement with young people, creating opportunities for future development of processes and pathways within diabetes care.
- Work alongside the paediatric and adult diabetes team to jointly develop and deliver an agreed project plan focused on Transition of care from paediatric to adult health services. Working to meet the requirements of funders and local team priorities, that is in line with UHS processes and policies.
- To act as an advocate and source of support for young people whose care is maintained under the RACH Children and Young Person’s Diabetes Team, and the relevant adult services.
- To work flexibly to assist with the provision of a varied programme of personal, social and educational developmental activities with due regard for the needs and opinions of young people. This may include evenings, weekends.

- Registered Youth Work degree or equivalent Professional Qualification
- Experience of working with young people in a variety of settings
- Manage workload independently
- IT skills: PowerPoint, Excel, able to access and search the internet for appropriate evidence. Able to use E-mail Data entry skills.

- Experience of working with young people in a healthcare/hospital setting.
- Experience of working with young people with long-term health conditions and their families.
- Able to drive for a variety of distances.
