Job overview

The Patient Flow Team are looking for an enthusiastic and experienced nurse to join the existing team of In-Reach Practitioners  that support with assessing patients at the acute hospitals in Brighton and Haywards Heath, ascertaining their suitability for inpatient rehabilitation and supporting with the Discharge to Assess Model.

Main duties of the job

Full training will be provided, but experience in patient assessment and discharge pathways would be an advantage.

This role will require a driving licence and access to your own transport as you will be working across sites.

Detailed job description and main responsibilities

a) The In-Reach Practitioner will support the System Capacity and Flow
Manager in managing patient flow from acute providers to and across SCFT
Intermediate Care Units.
b) Ensure all bed capacity across the Trust is utilised whilst ensuring
patients experience safe, high quality care throughout their patient
journey.
c) Work with system partners and colleagues to expedite safe transfers
and provide safe alternative pathways for patients.
d) To work collaboratively with external stakeholders and partner
organisations to address urgent care local pressure, system surges and
escalation ensuring capacity is fully utilised and patient flow is
maximised.

  • Current NMC Registration or other Health Care Professional Qualification
  • Degree or equivalent experience
  • Evidence of continuous professional development
  • Relevant post graduate level qualification or equivalent experience
