Job overview

To support the NY&Y Transformation Programme and to improve the care of people with a range of mental health needs accessing Primary Care Services, TEWV have been working in partnership with Primary Care Networks (PCNs) across North Yorkshire and York to recruit a growing number of First Contact Mental Health Practitioners (FCMHP), based in GP Practices. 

An exciting opportunity has arisen for a full time a Team Manager to join our expanding transformation team.  

      This is a full-time position supporting the FCMHP workforce embedded into the PCNs operating across the York and Selby geographic area, including City of York and Vale of York and Selby. 

We are looking for people with drive and passion, an ability to develop strong working relationships with internal and external partners, and to explore and promote new integrated ways of delivering proactive, personalised, coordinated and more integrated care for people closer to their homes, within their local communities.

This role is fundamental to the development of inclusive & integrated mental health services.

Main duties of the job

To continue with the recruitment of new roles for Primary Care in partnership with PCN colleagues.

To lead the induction and ongoing line management of the first contact workforce across the chosen geographic area. Working in partnership with the PCN to identify training and development needs for staff.

To develop and maintain robust clinical and operational governance to support these roles in practice.

To ensure the delivery of high quality and safe care in partnership with the PCNs and other stakeholders.

To continue to develop and maintain effective relationships with the PCNs and the Secondary Community Mental Health Services, ensuring consistency and good communication by being the single point of contact.

To encourage and develop further integration across Primary and Secondary Care services, creating opportunities for wider multidisciplinary working and support.

To ensure high standards of reporting, and to contribute to the co-creation of solutions for outcome monitoring across the system.

To participate and support the ongoing development of Community Mental Health Transformation Programme across North Yorkshire and York, which could include the design and development of local ‘hubs’.

Detailed job description and main responsibilities

To act as a role model for the team and demonstrates effective leadership through personal practice.

Responsible for the line management of all team members including appraisals, personal development planning and training, management of sickness absence, formal disciplinary and grievance matters, performance management, recruitment and selection decisions and high-level planning and prioritising of departmental workload.

The undertaking of some appraisals may be delegated as appropriate.

Responsible for ensuring the implementation of an effective system of clinical and management supervision, preceptorship, mentorship and training throughout the team to facilitate learning in practice. Participates directly in the provision of clinical supervision and training to team members as appropriate.

Responsible for ensuring that effective communication systems are maintained within the team and for strengthening partnership links with GPs, social workers, local authorities, statutory, private and voluntary independent providers of care, and other primary care agencies, attending meetings as required.

Communicates accurate and comprehensive operational information at management meetings using appropriate presentation skills, to inform decision making in the best interests of the service.

Communicates sensitive or contentious information to staff regarding e.g. disciplinary matters, organisational change where the atmosphere may be hostile or antagonistic but where the communication needs to be accepted or where co-operation needs to be achieved.

Participates in leadership network groups, professional forums, and special interest groups.

Analyses and interprets a range of complex facts and situations when assessing patient eligibility to access secondary care, or to determine the best course of action in particular circumstances. Judgements may be required where there exists a range of options.

Uses judgement when providing advice and guidance on complex clinical cases to other members of the nursing team and staff from other disciplines.

Uses judgement and analysis when investigating and responding to staff grievances and disciplinary/capability matters.

The post holder may be required to undertake duties not specified in the job description, but which are commensurate with the role and/or band as required by service need.

Person specification

Qualifications

Essential criteria
  • Current professional registration with appropriate body in healthcare or social work.
  • Evidence of Continuing Professional Development.
  • Certificate in teaching & assessing in clinical practice as appropriate to own profession.
  • Clinical Supervisor.
  • Leadership or management qualification or willingness to work towards. Qualification must be achieved within agreed timescale.
  • Quality Improvement Systems for Leaders or willing to undertake within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
Desirable criteria
  • Post-graduate diploma in a relevant area.

Experience

Essential criteria
  • Significant experience working with people with complex mental ill health in a community setting.
  • Significant experience of working collaboratively with service users and their families/carers
  • Significant experience of providing clinical supervision, mentoring and teaching to healthcare professionals and students.
  • Demonstrable leadership or management experience
  • Significant role in quality improvement activities.
Desirable criteria
  • Use of the Trust’s Quality Improvement system

Knowledge

Essential criteria
  • Significant evidenced based knowledge, to post-graduate level, of best practice in caring for patients in the designated field.
  • Detailed understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act) and its application in practice.
  • Detailed understanding of clinical governance and its application in practice.
  • Detailed understanding of Safeguarding and its application in practice.
  • Detailed understanding of Care Programme Approach and its application in practice
