With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

We have an exciting opportunity available for an Advanced Clinical  Pharmacist to join our team and specialise in this area. 

You will lead on all aspects of medicines optimisation and medication use within the specialism of elderly care, supporting medical and nursing teams, as well as allied health professionals in the safe and effective use of medicines. 

Pharmacy and Medicines Optimisation at the Trust are well developed and we are committed to sharing excellence through our clinical services and teaching roles.  The department is committed to the use of new technology using ASCribe/EMIS, Omnicell electronic storage cabinets, ChemoCare and TrakCare electronic prescribing systems and two state of the art BD Rowa Vmax robots. 

We will support you to continue to develop your advanced skills in clinical pharmacy, and would expect you to have a significant impact on the use of medicines by supporting safe and high quality services and supporting the patient journey.

Working within and leading a small pharmacy team, you will use appropriate skill mix within the team to ensure that medicines reconciliations are completed in a timely manner, in line with current national recommendations, safe and efficient use of patient's own medication and effective communication through transfers of care.

Through excellent leadership skills, you will be expected to implement changes required to ensure that the service runs in a safe, efficient and cost-effective way.

Specific Duties

1.    Provide an advanced and highly specialised clinical pharmacy service to the Elderly Care wards.

2.  Lead on all aspects of medicines optimisation within the area of Elderly Care, liaising with key stakeholders to ensure use of safe, optimum and cost effective treatments, in line with national and local policies. 

3.    Contribute to the provision of audit and policy development in relevant areas, such as deprescribing, anticholinergic burden, etc.

4.   Deputise, where necessary, for advanced clinical pharmacists from relevant specialties, e.g. Stroke.

5.  Provide supervision to other members of the pharmacy team in a clinical environment.

6. Provide medicines reconciliation, review and care planning for individual patients with complex medication needs.

7.  Counsel patients in the correct use of medicines that have been prescribed and provide support when needed.

8.   Ensure seamless transfer of care through excellent communication and the use of electronic resources, e.g. PharmOutcomes

9. Collect, manipulate and analyse data on the qualitative and quantitative aspects of the effects of the Directorate Pharmacist, by participating in intervention monitoring, encouraging the reporting of medicines errors and undertaking practice research. 

10. Assist in the development of policies, guidelines and pathways relating to drug therapies.

11.  Undertake the professional duties of a pharmacist in order to ensure that the medicine use process conforms to legal, ethical and moral guidelines accepted by the General Pharmaceutical Council and Royal Pharmaceutical Society.

12.  Be an active member of the ward-based multi-disciplinary care team.

13. Represent Pharmacy at directorate/service meetings relevant to clinical area of practice. 

14.  Provide advice on medicines use and medicines expenditure to the Directorate.

15.  Liaise with Pharmacy section heads on matters of mutual interest.

16.  Contribute to the clinical development of all staff by participating in assessments of need, proficiency and the delivery of training.

17.  Provide directive teaching, small group work and on-ward teaching rounds to medical undergraduates, junior medical staff and other health care practitioners.

General Duties

2. Participate in local, regional or national forums/meetings/working parties.

3.    To promote a safety conscious culture. 

4.  To contribute to the development of the pharmacy service at North Tees and Hartlepool NHS Trust.

5. To contribute to achieving the in year performance targets and objectives.

6.    Support junior pharmacy staff in performing their duties.

7.    Keep up to date with practice through CPD.

8.    Act as a designated and authorised Pharmacy key holder.

9. Demonstrate commitment to IWL principles and flexible working patterns, to meet the needs of the service and staff.

10. Tackle discrimination and harassment and promote equality and diversity in the workplace.

11.  Reduce sickness absence; work place accidents; and promote zero tolerance on violence against staff.

12.  Take responsibility for personal development and education and the development of a Personal Development Plan.

13. Be a designated line manager for a junior pharmacist(s) and/or technician. This will include responsibility for carrying out regular appraisals and one to ones, managing sickness absence and ensuring business goals are clearly communicated and linked to objectives.

14.  Review training and development of staff, so all staff have a personal development plan and support staff in reaching their full potential through the appraisal process.

15.  To act as the Responsible Pharmacist when covering the In-patient or Out-Patient Dispensaries in the Trust, which includes supervising Over the Counter Medicines sales direct to patients, using your skills in managing minor ailments.

16.  To act as an Independent Prescriber (once qualified) to provide direct care to patients by means of any Trust prescribing stationery.
