Job overview

Dorset HealthCare are currently seeking a Community Staff Nurse to join our vibrant and innovative Integrated Care Team in Purbeck, in the District and Community Nurse team in Wareham.

You would work alongside our experienced Community Nurse Teams helping to provide care needs to a high-quality standard of care for patients in their own homes.

We can be flexible with shift patterns as long as the service needs are met between hours of 08:00-20:00 with some attractive weekend enhanced pay.

We offer a structured induction programme, flexible working patterns and pastoral support. There are excellent learning and development opportunities, both in-house and through our links with local universities, plus the option of a preceptorship programme for newly qualified nurses to boost your skill development.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job

Main duties are to provide high-quality person-centred approach to care delivery which always considers people's safety, privacy, dignity whilst promoting and embodying the Trust’s values. Undertake assessment of patients, care needs, and develop programmes of care as part of the wider multi-disciplinary care team.

As a Community Staff Nurse, you would manage acute and chronic nursing needs, be able to develop strong, therapeutic relationships with your patients and work closely with community partners to achieve the best possible health outcomes.

You would be identifying care needs, monitoring patients’ conditions, recording observations and helping them develop their wellbeing care plans.

You’d be supporting people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital as well as delivering exceptional end of life care including symptom management and help in identifying and achieving people’s preferences.

Knowledge, Skills and Training

Essential criteria
  • Registered practitioner to degree/diploma level
  • Membership of the relevant Professional Body
  • Evidence of recent professional development in an up to date portfolio
  • Knowledge and understanding of Trust Strategy and policies relevant to role
Desirable criteria
  • Demonstrates a knowledge ofsynergy with Trust vision, values and behaviours

Job Specific Experience

Essential criteria
  • Supervised experience gained during clinical placements
  • Ability to contribute to student development / supervise other junior staff
  • Demonstrable knowledge of assessment and therapeutic interventions in area of practice
  • Experience of contributing to the management of change
Desirable criteria
  • Recent previous experience within a comparable role
  • student assessor

Managerial Experience

Essential criteria
  • Experience of demonstrating duties to junior staff
  • Ability to delegate care safely
  • Is prepared and skilled to act up and/or co-ordinate shifts in the absence of the team lead
Desirable criteria
  • Experience of providing clinical supervision and mentoring to junior staff

Information Technology

Essential criteria
  • Able to evaluate data and produce reports using Microsoft Excel and Word
  • Experience of using electronic patient / service user record systems

  • Is happy working in an agile way
