An exciting opportunity has arisen for an enthusiastic and organised person to join our Health and Social Care Co-ordination Team within the cluster of East Dorset, based at St Leonards Hospital.  The role is paramount in bringing services together in order for them to provide seamless care and support for frail and vulnerable individuals living in our cluster. The role involves supporting patients in the community and those being discharged from hospital. It is an exciting role at the forefront of developments in Community Services and integrated working.

Experience in a health or social care environment would be beneficial. You must have excellent administration and communication skills; as part of you role will be minute taking and chairing meetings. You will be working in partnership with primary care, community services, local authority, acute trusts and voluntary services; supporting other professionals, by effectively providing accurate up to date relevant information by using a wide range of clinical systems.   

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

This role is predominantly office based.

Prif ddyletswyddau'r swydd

This role is seen as a critical and evolving post to support the locality based Multi-Disciplinary Teams (MDTs) within East Dorset  to deliver effective, co-ordinated care for those individuals identified as have very high or high intensity and complex needs and who are at high risk of a hospital emergency admission, ED attendances or out of hours care. Facilitation and support with the MDT for hospital admissions and discharges to ensure appropriate community engagement to minimise risk of readmission.

To work closely with health and social care teams to:

  • Identifying individuals who would benefit from care coordination and/or case-management
  • Provide effective co-ordination of the multi-disciplinary team (MDT) meetings and follow through on agreed actions, engaging the relevant teams/individuals as necessary including the Over 75s and Frailty teams.
  • To access and monitor Carer Registers and alert relevant professionals where safeguarding risks are identified.
  • Provide a care-navigation function to enable individuals to access a range of third sector and support services.
  • To have access to Primary, Community and Social Care systems to facilitate effective multi-agency communication.
  • To be a congruent between community, social services and hospital colleagues
