Advanced Clinical Practitioner (ACP) in Multiple Sclerosis - Band 8a - 37.5 hours per week (to be worked Monday to Friday 08.30-16.30)

An exciting opportunity has arisen for a highly motivated and dynamic nurse or allied health professional (AHP) to join the multi-disciplinary Dorset Multiple Sclerosis Service, in this newly created post.

We are looking for an enthusiastic and dedicated registered nurse or AHP with an ACP MSc and non-medical prescribing qualification.  The successful candidate will have excellent communication skills and relevant experience to deliver high quality, specialist care to people with MS in the Dorset area, and to be part of the on-going development of this forward-thinking service.  

You will work within the hospital and community settings across Dorset.

We are committed to continuous professional development and offer on-going training within the MS Service.

Car driver essential (or the ability to travel to meet the needs of the role - not all locations are accessible by public transport) .

Based at Poole with cross site working

Main duties of the job

The ACP will be responsible for the assessment, diagnostics, and treatment of individuals on the Dorset Multiple Sclerosis Service caseload using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience; this will include the need for complex history taking, neurological physical examination, prescribing (including for high-cost disease modifying drugs) and non-medical requesting of investigations.

Detailed job description and main responsibilities

The post holder will:

  • Assume responsibility for the daily management of the clinical areas to which they have been designated.
  • Hold a clinical caseload alongside other nurses/practitioners in the MS Service
  • Provide an ACP Nurse/Practitioner service to designated clinics; this will include: ACP led MS disease modifying drug (DMD) and MS urgent assessment clinics, MS Nurse review and DMD clinics, the MS Service helpline, and MDT MS acute symptom clinics.
  • Conduct home visits for holistic assessment, management, and coordination of care for those with advanced and/or complex MS
  • Prescribe medication for individual patients within the agreed scope of practice; this will include repeat prescribing of MS DMDs and symptomatic treatments.
  • Participate in team meetings, including DMD MDT meetings.
  • Provide clinics for educating and consenting of new treatments, reviewing patients for drug interactions, dose adjustments, and new starters/switches 3-monthly reviews as part of the MDT
  • Lead on clinical governance; repeat prescribing, DMD safety monitoring, and protocol development in MS as part of the MS MDT
  • Ensure NHSE blueteq forms are completed annually for high cost DMDs where the ACP is the prescriber.
  • Manage new MS drug therapies introduction, linking with pharmacy to ensure these are added to the formulary, and leading on relevant NICE TA’s and as part of the MDT.
  • Provide clinical supervision to other MS Specialist Nurses/Practitioners in the service
  • To take responsibility for DMD safety monitoring for the team in service coordinator’s absence
  • Take delegated responsibility from the service lead, including deputising for the service lead as required.

Essential criteria
  • Professional registration with relevant professional body
  • Masters level Advanced Clinical Practice/ or Equivalent Masters (or experience ratified through credentialling process)
  • Non-Medical Prescriber

Experience

Essential criteria
  • Demonstrable experience of working independently in a band 7 clinical role within the field of Multiple Sclerosis
  • Demonstrable theoretical and practical knowledge of symptom assessment and management in MS.
  • Demonstrable theoretical and practical knowledge of disease modifying treatments for Multiple Sclerosis
  • Developing policies, protocols and standard operating procedures

Technical Skills and Competencies

Essential criteria
  • Neurological physical examination skills
  • Advanced communication skills for difficult conversations, (such as: discussions regarding end of life, sexual dysfunction, permanent disability, and with senior colleagues where there may a difference in opinion).
  • Ability to analyse and interpret complex information from patient assessment and interventions, including clinical history, physical assessment, pathology and microbiology tests, and MRI scans/reports.
  • Clinical reasoning, problem-solving and decision-making skills at an advanced level
  • Demonstrable ability to work in a busy environment and resilience when the service is under pressure.
  • Ability to drive and has access to a car for work purposes
