Job overview
At UHD, we are dedicated to providing exceptional care to our elderly population. Our Same Day Emergency Care (SDEC) clinics within Older People's Services are currently based on the Poole and Bournemouth Hospital sites and are at the forefront of delivering timely and effective interventions, to ensure our patients receive the best possible outcomes. We pride ourselves on delivering comprehensive, compassionate and patient centred care for each individual and are seeking a dynamic and experienced Advanced Clinical Practitioner (ACP) to join our multidisciplinary team.
The ACP will play a pivotal role in the same day emergency care clinic, providing high-level clinical expertise and delivery of high quality care to older people. The successful candidate will be responsible for managing complex cases, undertaking comprehensive geriatric assessments, ordering and interpreting investigations and formulating patient focused management plans.
The successful post holder will hold an accredited MSc in Advanced Clinical Practice or have completed the e-portfolio route with digital badge and be willing to complete our local competency booklet evidencing proficiency in the care of Older People.

There is a planned move for this service anticipated during Autumn 2025, when this role and base of work will relocate to Bournemouth site. Upon relocation of the role/service, excess mileage allowance / reimbursement will not apply, and any travel between the post holder’s home and work base will be classified as a commute. Any other changes unrelated to this, will be handled in line with Trust/National terms and conditions.

- Recognised teaching or mentoring qualification

- Demonstrable theoretical and practical knowledge of OPS specialty common presentations
- Participation in research and experience of critical appraisal of the evidence base to inform best practice
