An opportunity has arisen to work in an exciting integrated model for the delivery of patient care in Dorset County Hospital’s Acute Hospital at Home service. This  service proactively manages and monitors a defined caseload of patients who have complex care needs in the short term, on a multi-disciplinary basis in their own home.

We are looking for a dynamic Healthcare Assistant/Therapy Assistant, to work in our multi-disciplinary team, ensuring a patient focused and individualised approach. Ideally you will have acute and/or community experience within a multi-disciplinary team working .

 Car Driver is essential for this post.

Our unique Acute Hospital at Home team are looking to recruit Healthcare Assistant/Therapy Assistants. Our service proactively manages and monitors a defined caseload of patients, who have complex care short term needs. The team is truly multidisciplinary, and consists of Therapists, Staff Nurses and HCA/Therapy Assistants.

Our team are in the privileged position of treating people in their own home, getting to know them well and seeing them recover from acute illness. Our team and our service work with the patient’s needs at its heart and aims to support admission avoidance and early discharge from hospital wards. The focus of the service is the acutely medically ill.

We are looking for a team member who:

- Has 12 months experience in a healthcare setting

- Has a special interest and passion for caring for the patients of our community

- Has evidence of and/or insight into community working

Patients in receipt of Acute Hospital at Home treatment are offered continuity of care by following them from hospital into the community and the post provides the opportunity to work  closely with acute, community and primary care colleagues in a seamless service. It is truly rewarding watching patients recover In their own home. 

Acute Hospital at Home staff will experience a unique opportunity to interact with senior, specialist nurses and medical staff with the support of their line managers. There are opportunitites to develop a number of nursing and therapy based skills. 

This is a service with the patient’s needs at its heart and aims to support admission avoidance and early discharge.

To ensure quality care and support service which enables patients to be supported throughout their treatment pathway whilst promoting independence and protecting choice, dignity, privacy and safety in their own homes.

To work in support of all qualified staff in the assessment, planning, implementation and evaluation of treatment with and without direct supervision of a registered practitioner i.e. you will be supported to work independently to treat, assess and advocate for your patients.

To work as an independent member of staff to ensure that holistic care is delivered, and standards of treatment are high.

To communicate effectively at all times within the team, other health care professionals and with the patient family/carers.

Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

To regularly assess evaluate and report on actions and activities to registered staff.

To supervise and delegate work to junior members of the team.

Act as a resource and role model to others involved in the Acute Hospital at Home Service

Participate in the development, planning and delivery of the ever-evolving Acute Hospital at Home Service.

EDUCATION, QUALIFICATIONS

  • English Functional skills level 1/ GCSE D-G
  • Maths Functional skills level 1/ GCSE D-G

  • NVQ Level 2 in Health Care or related field.
  • Completion of the Care Certificate.

EXPERIENCE

  • A minimum of 12 months experience within a healthcare setting, either,- Nursing Home experience Acute Hospital experience,Therapy/Rehab Experience

SKILLS, ABILITIES & KNOWLEDGE

  • Evidence of good communication skills.
  • Demonstrate an understanding of confidentiality.
  • Evidence of ability to maintain accurate and legible documentation.
  • Ability to work as part of a team.
  • Ability to Drive – (clean licence) over 12 months qualified

  • Evidence of ability to maintain effective working relationships with colleagues, patients and visitors.
  • Have an awareness for Health & Safety
  • Awareness of own developmental needs within the post
