·       Under the supervision of the live donor coordinators be responsible for his/her own caseload of donors entering the live donor programmed whilst working as part of the Multidisciplinary team.

·       To provide information and support to people coming forward as potential living kidney donors identifying their psychological, physiological, emotional, spiritual and cultural needs.

·       To conduct initial health screening of potential donors and facilitate recruitment and triage of referrals.

·       To provide education about the donation process including compatibility testing, investigations/screening, the surgical procedure, the legal approval and aftercare involved to enable the donor to make an informed choice.

·        Provide annual review for donors who have already donated based on national polices and standards. Keep up to date in changing guideline’s and evaluate the service provided.

·       Assist the live donor coordinators in the co-ordination of the kidney sharing schemes.

·       To undertake social and physical assessment of the potential donor maintaining appropriate documentation on vitaldata and in the hospital records. To request relevant investigations following the living donor pathway, coordinating and facilitating the one stop live donor clinic to streamline the work up process.

• Manage and coordinate new and existing referrals for patients who are suitable for transplantation, enabling discussion in the multidisciplinary team meeting and liaising with the referral centres.
• Maintain the transplant waiting list by regularly reviewing the active, suspended and pending lists facilitating regular meetings with the referring consultants to ensure suitability and enhance timely activation or reactivation.
• Prepare and coordinate transplant assessment clinics locally and regionally, providing an education session for potential recipients and their families.
• To ensure patients who are on the transplant waiting list are annually consented, keeping their contact details up to date, confirming the process of accepting an offer and revisiting the option of live donation.
• Commence education programme for new transplant recipients following agreed educational packages and guidelines. Provide education for renal transplant patients and their families/carers pre and post transplantation. Provide psychological support pre and post transplantation. Effectively triage both routine and acute referrals being a first point of contact for transplant patients. Triage telephone enquiries and manage the day to day running of the transplant office. Take part in the in- reach service.
• Provide nurse led clinics for new and established patients in both routine and emergency situations.
• Coordinate and facilitate anaemia management of transplant patients including intravenous iron administration and monitoring and review of erythropoietin.
• Provide annual reviews for post-transplant patients based on national polices and standards. Keep up to date in changing guidelines and evaluate the service provided. Provide screening and health education, both within Derriford hospital and travelling to other areas to provide an outreach service.
• Provide advice and support to patients who have a failing transplant, facilitating timely referral back to the chronic kidney disease team for appropriate education.

• Participate in a 1:6 on-call rota to cover donor referrals and kidney offers from NHSBT, 24 hours a day, 365 days a year, coordinate the process from initial referral to the admission of the patient. Liaise and refer to all relevant departments and members of the multi-disciplinary team to deliver a planned and seamless service.
• To undertake venepuncture following hospital trust guidelines.
• To communicate effectively with other organisations and healthcare professionals including but not limited to general practitioners, referring centres and regional transplant units.
• Coordinate patient investigations and results for review by medical practitioners.
• Complete UKT documentation for the pre, post-transplant patients and live donors. To maintain clear and accurate records using the vitaldata database ensuring the appropriate use of all clinical systems.
• Carry out basic statistical analysis on data collected.
• Present data at internal and external meetings.
• To complete the transplant nurse practitioner/ live donor competencies.
• To complete all NHSBT documentation.
• To communicate effectively with other organisations and healthcare professionals including but not limited to general practitioners, referring centres and regional transplant units.
• To maintain their own professional development, attending courses/further training to enhance and optimise his/her role within the work area.
• To participate in the training and development of junior staff on the ward and participate in the supervision and support of student nurses and new members of staff joining the renal unit.
• To promote and safeguard the wellbeing of patients, their families, and colleagues.
• To participate in the development and implementation of ward area policy and nursing strategies.
• To respect the confidential information, in line with Trust policy Data Protection Act, obtained in the course of work and not disclose this information.
• To be familiar with and adhere to Trust and Departmental policies and procedures mindful of responsibilities to self, colleagues and patients.
