Senior CNS for Parkinson’s DAT and Service Nurse Lead

Job overview

1) To act as the lead nurse of the University Hospitals Plymouth Parkinson’s disease service (0.4 FTE)

0.4 FTE of this role will be to provide strategic leadership for UHPNT Parkinson’s disease service and direct line management for a rapidly growing team of Parkinson’s specialist practitioners/nurses. This includes a nurse-led new patient pathway for newly diagnosed PD, a nurse-led pathway for supported self-management and patient-initiated follow-up, a nurse-led inpatient liaison service,  a regional atypical Parkinson’s clinic and a rapidly expanding device assisted therapy (DAT) service.  Part of this role will be the direct line management of a number of specialist practitioners.

2) To act as a Senior CNS for device assisted therapies (0.6 FTE)

0.6 FTE will be to provide clinical care within and service development of the device assisted therapies (DAT) service at University Hospitals Plymouth NHS Trust. Funding has recently been obtained to expand this service to build a comprehensive care pathway for those with advanced/complex Parkinson’s disease. This will allow equitable access to infusion based dopaminergic treatments (apomorphine, foslevodopa-foscarbidopa and levodopa/carbidopa intestinal gel) and Deep Brain Stimulation within the catchment area of UHPNT. It forms part of a regional network of Parkinson’s DAT nurses, operating across the South West Peninsula, in close collaboration with the established regional DAT centre at North Bristol NHS Trust.

 

Main duties of the job

UHPNT will be the first centre across the South West Peninsula to run a dedicated DAT clinic; it is anticipated that satellite clinics will in due course open at other acute NHS Trusts across the South West Peninsula. Part of the role will be to provide support for Parkinson’s nurse specialists to establish and run clinics at these centres. This will entail a strong element of regional education to facilitate knowledge transfer and sharing of best practice.

Both aspects of the role are anticipated to be predominately hospital based. Most clinical contact will be in person, either at UHPNT sites or within the community (including where appropriate at the patient’s home). There may be a limited opportunity for some home working, however this would be anticipated to be largely restricted to administrative or service development tasks.

Detailed job description and main responsibilities

Clinical

        Demonstrate advanced empathetic interpersonal and communication skills in dealing with patients and carers from the point of diagnosis, supporting people in making decisions, planning care or seeking to make positive changes, using Health Education England’s framework to promote person-centred approaches in health and care.

        Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for patients by virtue of expert clinical judgement and close liaison with members of the MDT.

        Demonstrate advanced practice in holistic assessment, planning, implementation and evaluation of patient care. This will necessitate high levels of autonomy and advocacy.

        Demonstrate advanced knowledge, skills and innovative practice in relevant specialist area. Promote clinical excellence. Demonstrate continual evaluation of practice, making agreed changes and evaluating their impact.

        Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

       Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate.

 

Education / Training

        Responsively lead on driving an education agenda which facilitates the learning of the wider workforce, especially when key areas of growth are identified.

        Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice.

        Facilitate collaboration of the wider team and support peer review processes to identify individual and team learning.

        Supporting the wider team to build capacity and capability through work-based and inter- professional learning, and the application of learning to practice.

 

Person specification

Qualifications

Essential criteria
  • First level nurse
  • ENB 998 (Teaching and Assessing in Clinical Practice /D33/D32), or equivalent course
  • NMC registration
  • Must possess, or be willing to work towards a Masters degree in related area
  • Independent non-medical prescribing
  • Proven evidence of relevant, recent study and continuing professional development in Parkinson’s care
Desirable criteria
  • Masters degree in in related area
  • Leadership qualification
  • Have undertaken an accredited Leadership course
  • Education/Mentorship Course
  • Neurology Academy Masterclass in Parkinson’s
  • Degree module in Parkinson’s

  • Extensive demonstrable experience within Parkinson’s care (to include all aspects of care – in-patient, out-patient, device-aided therapies and atypical Parkinson’s syndromes) and at all disease stages
  • Theoretical and practical in depth proven knowledge of Parkinson’s disease and related disorders
  • Expert proven knowledge and understanding of the use of levodopa, dopamine agonists, COMTI and MAOI
  • Proven track record of autonomously managing people with Parkinson’s at all stages of the disease
  • Proven knowledge and ability to take an accurate, detailed clinical history and willing to undertake a formal qualification in history taking
