Job overview

University Hospitals Plymouth NHS Trust are recruiting Consultants in Diabetes and Endocrinology.

There are eleven consultants working in Diabetes and Endocrinology. This is an enthusiastic and dynamic department providing the full range of secondary care diabetes and endocrine services, the full gamut of dynamic endocrine testing, but also community diabetes and supporting a number of tertiary services including pituitary, thyroid, parathyroid and adrenal surgery, joint pancreas and kidney transplantation, and a neuro-endocrine tumour service.

This vacancy is for a physician who is accredited in Diabetes and Endocrinology/ Acute and General Internal Medicine or is in the final stages (6 months or less) from obtaining full accreditation. We are looking for applicants with drive and enthusiasm who are committed to a career in diabetes and endocrinology and continuing to develop the service we provide our patients in the future.

Main duties of the job

The full range of sub-speciality clinics are provided. Opportunities to contribute to all sub-speciality areas are negotiable. The department operates weekly joint insulin pump clinics, Joint Diabetes Foot clinic with vascular, orthopaedic, and orthotics, joint renal / diabetes clinics, with twice monthly diabetic nephropathy clinics, fortnightly joint Adolescent Diabetic and Endocrine clinic with a Consultant Paediatrician, joint thyroid eye clinic with consultant Ophthalmologist, joint fertility clinic with Obs & Gynae specialist, joint pituitary clinics with pituitary surgeon, Joint Adrenal Incidentaloma virtual clinics and weight management clinic at Cumberland centre. Candidates would be encouraged to develop areas of sub-specialty expertise complementary to those within the department.

The hospital has established a 23-bed short stay ward. This unit is managed by the Diabetes/ Endocrine team. The short stay ward provides a hub for management of all medical patients with a short length of stay with input from all medical specialities. The team will supervise this unit on a 7-week rolling rota.

 

During the week on inpatient service the consultant will also support the inpatient diabetes nursing team with the help of a specialist registrar and provide inpatient diabetes and endocrine speciality advice.

Detailed job description and main responsibilities

The appointee will be expected to carry out the clinical timetable for Consultant in Diabetes and Endocrinology which will be negotiable. The successful candidate will provide speciality as well as GIM workload. There will be 1:7-week rota contribution towards the Short Stay Ward and 1:7 for inpatient diabetes and endocrine speciality cover within the trust.

The rest of time 5:7 will be allocated to speciality work intended to enhance the delivery of Diabetes & Endocrinology care including outpatient and community services to the local population in Devon and Cornwall. In addition, there will be an expectation that patients on outlying medical wards will also be seen by the appointee for a number of weeks per year.

The Plymouth and West Devon Weight Management Service is provided by Livewell Southwest. This is a large, well-established, innovative and forward-looking multidisciplinary service. An interest in, and some experience of bariatric medicine would be an advantage, because part of this post could optionally involve contributing to the provision of medical support for the assessment and management of complex obesity and its comorbidities, including evaluation prior to, and metabolic and biochemical monitoring after bariatric surgery. The team is also research active and were recently awarded a £2.5M programme grant by NIHR, with UHP as sponsor and host. The service is an integrated Tier 3 and Tier 4 service which works across the Livewell / UHP boundary and Tier 2 community weight management is now being integrated into the same pathway. There would be opportunities for suitable appointees to contribute to the development of a range of aspects of the service, as well as to teaching and research.

In line with a basic 10 PA job plan, further opportunities will be available to negotiate additional PAs related to specific sub-speciality interests; this could also potentially include contributions to the general medicine team within UHP. An indicative and negotiable 10 PA Job Plan is attached.

In addition to general Diabetes Clinics, specialist clinics including: - insulin pump clinics, community diabetes clinics in Liskeard Hospital, Ante Natal / Diabetic clinics with Dr I Montague (Consultant Obstetrician), Joint Diabetes Foot clinic with vascular, orthopaedic, and orthotics, joint renal / diabetes clinics, with twice monthly diabetic nephropathy clinics, fortnightly joint Adolescent Diabetic clinic is held with a Consultant Paediatrician, joint thyroid eye clinic with consultant Ophthalmologist, joint fertility clinic with Obs & Gynae specialist, joint pituitary clinics with pituitary surgeon, Adrenal Incidentaloma virtual clinics and weight management clinic at Cumberland centre. Candidates would be encouraged to develop an area of expertise complementary to those within the department.

The endocrine service incorporates pituitary, thyroid, parathyroid and adrenal surgery on site, chemical pathology with a full range of endocrine assays and dedicated endocrine nurses for dynamic function testing in the Diabetes Centre. A DEXA bone densitometer is available for osteoporosis management.

The newly appointed consultant will be offered a mentoring opportunity, and this will be facilitated through discussion with the clinical director. Within the first six months there will be a job plan review. The need for additional DCC or SPA time will be discussed at that point with the service line lead.

  • Successfully completed an ATLS and APLS provider course.
  • Level 3 child protection trianing.
  • To be on the appropriate GMC specialist register or eligible for admission within six months of interview.

Experience

  • Demonstrate evidence of good general training.
