Job overview

NHS Devon ICB seeks to recruit an experienced individual to support in the delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.

This role will provide professional leadership, advice and expertise to ensure delivery of a high-quality standard of clinical assessment, review and management of CHC and joint funded cases working collaboratively with key partners to achieve best outcomes.

We will consider substantive and secondment applications.


The post holder will provide advice, support, supervision, and line management to a range of practitioners, ensuring that they deliver the highest quality and most appropriate services to patients whilst driving continuous service improvement.

This is an exciting opportunity for an enthusiastic and CHC experienced registered health or social care professional. We welcome AHP's, Social Workers and nurses from multiple disciplines who are looking to move into a more senior role within AACC.

Main duties of the job

Our aim is to ensure individuals encounter improved experiences, transparency, and consistency across all forms of continuing care, with a smooth transition between services and resource provision.

It is important that everyone receives an equal and comparable, good quality, consistent, high standard, person-centred experience.

As
senior specialist  you will work under the guidance and support of a Principal Quality Assurance Lead to implement the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.

You will support the high-quality delivery of clinical assessments and case management, working collaboratively with key partners to achieve best outcomes.

You will need excellent communication and assessment skills, be able to provide expert CHC advice and guidance to support staff involved with the assessment, planning and management of patients with complex health needs.

As well as provide line-management to AACC staff, ensuring that they have the support that they require to deliver the highest quality and appropriate services to patients and to drive continuous service improvement.

Successful candidates will need to have a flexible attitude and be a team player who enjoys working within a fast-paced service.

Experience, Knowledge & Qualifications

Essential criteria
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area.
  • Relevant post graduate degree or equivalent level of experience.
  • Experience of working within the All Age Continuing Care pathway.
  • Significant experience of successfully operating in a politically sensitive environment.
  • Experience of managing risks and reporting.
  • Experience of drafting briefing papers and correspondence at a senior level.

  • Demonstrated experience in a Healthcare environment.
