We have an exciting opportunity to recruit to the post of 'Enhanced Clinical Practitioner' to work in the community looking after our frail and elderly patients.
- The role is full-time permanent position working 37.5 hours per week.
Must have completed both level 7 modules of APACS/CDM and Non-Medical Prescribing qualification and completion of E-portfolio to be uplifted from a trainee band 6 to a qualified band 7.
Or – able to evidence all aspects of person spec if already in same role
Main duties of the job
· To provide expert enhanced clinical management for patients who are frail and/or with multiple long-term conditions who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission, or unnecessary length of hospital stay.
· To provide enhanced level clinical assessment, make a diagnosis, initiate appropriate clinical treatment, refer appropriately and lead to co-ordination of care, working with partner agencies and key professionals.
· Investigate and diagnose an unwell individual with frailty and or long term conditions in their own homes and in care homes.
· Reduction of unplanned admissions when appropriate
· Management of those with long term conditions including frailty
· High quality care to patients and families
· Managing complex medical conditions including long term conditions and frailty
· Carry out acute one-off visits when required (within own skill set)
· Dealing with challenging behaviour
· Working with existing resources
· Dealing with vulnerable clients and families
· Developing own clinical skills

Education and Qualifications
Essential criteria
- RGN or HCPC accredited degree that can complete the additional post registration training to V300 Independent non-medical prescribers
- Registered with NMC (Nursing and Midwifery Council)
- Evidence of post registration clinical professional development
- Independent Nurse Prescriber
Desirable criteria
- Enhanced Clinical decision making and patient examination skills

Essential criteria
- High level of understanding regarding long term conditions agenda and wider NHS agenda
- Working automonously in an advanced nursing role

- Experience or training in personalised care and support planning

Essential criteria
- Excellent communication skills both written and verbal
- Management of risk and health and safety and safeguarding adults and children procedures and ability to implement
- High level clinical skills relevant to long term conditions and the management of chronic disease management and frailty
- Advanced clinical practice skills
- Management of patients with complex needs
- Accountability of own role and other roles in a primary care setting
- Knowledge of practical application of clinical governance
- Willingness to work flexible hours when required to meet work demands
- Access to own transport
- Ability to travel across the locality on a regular basis
