Job overview

Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills with the ability to assess and lead on clinical decision making?

 If so, we would really like to hear from you.

This is an exciting role, working in a fast paced working environment within a well-established, multi-disciplinary team. The crisis and home treatment teams offer a high quality assessment and home treatment service as an alternative to hospital admission, to individuals experiencing acute mental health crisis.

As a service we are looking for individuals who are genuinely warm and engaging and focussed on client / customer satisfaction.           

If you are applying for a secondment you must obtain prior approval from your current line manager before submitting your application.

Main duties of the job

  • To provide first line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
  • To be able to rapidly assess people with mental health problems who are vulnerable to hospital admission
  • To provide specialist crisis assessments looking at the focus being on “hospital at home".
  • To co-ordinate the Crisis and Home Treatment Team on a shift by shift basis, acting as Duty Worker to triage referrals, plan the shifts activities and team accordingly.
  • To participate in team MDMs.
