Community Healthcare Support Worker - CRT

The Community Response Team supports discharges from the acute hospitals, providing short-term and interim care. The team will also provide re-enablement to suitable patients to improve independence. Previous caring experience is ESSENTIAL.

You will be based at Babington Hospital in Belper to cover the localities of Amber Valley and Erewash. You will need to be able to travel between visits, therefore a full UK driving licence and access to a vehicle is essential. 

You will need to be flexible and committed to caring and supporting people at home with care needs. Shifts follow a 7 day rota, including weekends and bank holidays. The team work early (08:00 - 14:00) and late (16:00 - 22:00) shifts and will have set allocated visits plotted.

Main duties of the job

Community Healthcare Support Worker - CRT

  • Attending allocated visits in the Community to provide care to patients and/or re-enablement as required.
  • Use of a toughbook and Systmone to follow set care plans and document notes from the visit. 
  • Communicate effectively with patients, family members and carers.
  • Escalate concerns to the senior staff within the Community Response Team.
  • Attend team meetings and be an active member of the Community Response Team to develop strong working relationships.

Education, Qualifications and training

Essential criteria
  • BTEC Health and Social Care / NVQ level 3 in care or equivalent
  • Experience working in healthcare setting
Desirable criteria
  • Training / experience in palliative care, rehabilitation, care of older person

Experience and knowledge

Essential criteria
  • Working in Health or Social care environment
  • Understanding of the implications and responsibilities for risk management and safeguarding

Skills and attributes

Essential criteria
  • Able to travel across the county as essential for this role
  • Good literacy skills
Desirable criteria
  • UK driving licence and access to vehicle for work
