Band 3 Discharge Navigator band 3  37.5 hours per week(2 posts available)/22.5hr per week(1 post available)

The Transfer of Care Hub is a dedicated team who support the wards with patient discharges.

We are looking for a cheerful confident person to join our small team.

Communication is key in this role .

Working hours are  8-4 , 5 days a week including some weekends and bank holidays.

Main duties of the job

The role of the discharge navigator is to maintain patient flow within the hospital in line with the current government discharge guidelines.

The role is ward based and will include attending board rounds, ensuring all necessary actions have been performed to ensure discharge home in a timely manner.

You will play a key role in  providing the trust with their discharge information to enable adequate patient flow throughout the hospital.

As such we expect our  Discharge Navigators  to be organised and proactive with good communication skills enabling them to liaise with patients/families and all members of the multi disciplinary team. A basic understanding of IT is preferred.

You will be an effective independent worker who equally works well within a team environment.

The Trust is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

Standard Check: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.

Enhanced Check: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.

Qualifications

Essential criteria
  • GCSE maths and English grade C/4 or above
  • NVQ3 in care or Admin or equivalent experience or NVQ 3 in Care or equivalent experience

  • ECDL
