Job overview

An exciting and rare opportunity for a Team Lead in Furness Rapid Response has arisen. 

The successful candidate will require a good understanding of Community Services and have excellent leadership and management skills. 

Rapid Response services across Morecambe Bay are under significant development with a 2hr urgent community response mandate and the development of Virtual Wards.  The candidate will work alongside other Rapid Response Team Leads, Matron and Clinical Service Manager to embed these changes in services.

Main duties of the job

To manage the Furness Rapid Response Service, taking the lead role and being accountable for the performance, quality and clinical effectiveness of the team.

To manage the overall day to day operational workload for the team, ensuring timely appraisals, mandatory training compliance, sickness management and all relating human resource activity with the support of the HR team.

To manage a small and highly complex caseload of patients, who have diverse complexities of need utilising all appropriate resources effectively. This involves prioritising, assessing, co-ordinating, delivering and reviewing whole programmes of care delivered.

To ensure high standards of care is delivered to patients and their families/carers within their home environment or on the Step up/down units. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

To ensure service delivery and assessment of patient for admission avoidance in their home environment, acute hospital emergency department and to support rapid discharge.

To contribute to service development, support development of collaborative working and integration with health and social care providers.

Working for our organisation

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre. 

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. 

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. 

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Education

Essential criteria
  • Achieved grade/experience• Either; Level 1 registered nursing qualifications or HCPC recognised AHP Degree
  • Health Professions Council or NMC registered
  • Evidence of relevant post graduate education in related areas
Desirable criteria
  • People management skills training
  • Previous management courses
  • Evidence of post registration education

Experience

Essential criteria
  • Post graduate experience including community and acute settings
  • Evidence of being able to practice and manage complex patients and able to evaluate effectiveness of own practice
  • Evidence of relevant CPD.
  • Understanding of adult protection issues.
  • Good working knowledge of the Clinical Governance agenda.
  • Knowledge of other service providers.
  • Experience of audit
Desirable criteria
  • Previous senior position in related field
