Job overview

An exciting opportunity has arisen for an experienced and self-motivated nurse to join the Acute Care at Home Team based at Truro Health Park.

You will be part of a multi-disciplinary team delivering services to people in the community, striving to deliver high quality personalised care.  The aim of the service is to ensure that an integrated approach is taken when delivering care.

The post will deliver a range of skills and experience to all aspects of community care and will involve working with patients in their own homes or in the Acute and/or Community Hospital settings.

Main duties of the job

The post holder, as a Specialist Practice Community Acute Care Senior Staff Nurse, is responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge.

 The post holder will liaise with the referring hospital or GP when arranging admission to the scheme, and will take responsibility to effectively assess, develop, implement and evaluate patient care.

The Acute Care at Home Team work on a flexible rota shift system.  The service covers a period of 0800 hours to 2200 hours 7 days a week which includes an on-call system.  The geographical area when on duty is anywhere within Central and West and North Cornwall. The total population of this area is approximately 250,000.  The post holder is predominantly a lone worker within this area.

As part of the role you will regularly deputise in the absence of Team Leader and assist  in the management of the team, including the performance and development of staff and providing support to other team members.

Essential criteria
  • RGN 1st level registration or equivalent
  • Diploma level specialist training, experience, short courses (or equivalent experience)
Desirable criteria
  • Prescribing qualification
  • Teaching qualification, eg, City & Guilds, HEA Mentorship module (or working towards)
  • 1st Degree Diploma or working towards

Experience

  • Working without direct supervision, yet as a team member through experience and education
  • Substantial post registration experience in Acute or Community Team
  • Experience of a wide range of medical conditions
