This is an exciting opportunity to play an integral role in supporting a seamless patient pathway from admission to discharge.
You would be working with a friendly and dynamic team who are dedicated to providing excellent service to our patients and stakeholders by facilitating safe and timely discharges.
Prif ddyletswyddau'r swydd
- The post holder will have responsibility for actively participating as a key member of the multi-disciplinary team and have a leading role on discharge planning.
- The post holder will have an extensive knowledge of community services and will support development of new initiatives that support timely discharge.
- The post holder will support the wider team in identifying barriers to discharge and problem solving to overcome them to achieve positive results for the patient, their family, and carers.
- Supervise the ward based patient flow coordinators.
The post holder will work across several wards. The wards are spread over two hospital sites, Bodmin Hospital and Longreach House.
