The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. This is an exciting opportunity to play an integral role in supporting a seamless patient pathway from admission to discharge.

You would be working with a friendly and dynamic team who are dedicated to providing excellent service to our patients and stakeholders by facilitating safe and timely discharges.

Prif ddyletswyddau'r swydd

  • The post holder will have responsibility for actively participating as a key member of the multi-disciplinary team and have a leading role on discharge planning.
  • The post holder will have an extensive knowledge of community services and will support development of new initiatives that support timely discharge.
  • The post holder will support the wider team in identifying barriers to discharge and problem solving to overcome them to achieve positive results for the patient, their family, and carers.
  • Supervise the ward based patient flow coordinators.

The post holder will work across several wards. The wards are spread over two hospital sites, Bodmin Hospital and Longreach House.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health.  We also provide specialist support to people with dementia or a learning disability.

 Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon.  As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
