An exciting opportunity has arisen  for a full time community nurse to join our dynamic and committed community nursing teams situated in the beautiful rural area of Penwith.

We are looking for an experienced, caring, motivated community nurse to join our large, busy and professional  team covering several general practitioner surgeries in the Penwith area, your primary base being with the Hayle Community Team.

You should possess commitment to team working and innovative delivery of excellent standards of care. You will need to be able to demonstrate a real enthusiasm for community nursing as well as a desire for ongoing professional development. 

You should also have an understanding of the changing climate of care delivery in the community.

The ability to travel independently across the West Locality to support other teams and to fulfil the requirements of the post is essential, therefore access to a vehicle and a valid driving licence will be required to enable you to undertake the role of a community nurse.

Weekend, evening and bank holiday working will be a requirement of the post.

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.

To work in partnership with patients and carers to offer support and advice and promote informed choice.  

To support the role of the Community Nurse Team leader.

To work autonomously, providing effective, evidence based care in line with competency and training record and organisational policies, procedures and guidelines.

To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
