This is a unique partnership between Health and Social care in South Tees with a commitment to working together, improving the quality of our services and getting rid of existing barriers between health and social services.

Pulmonary Rehabilitation is receiving significant funding and development as part of the NHS Pulmonary Rehabilitation 5 year vision and is an excellent time to join this team and contribute to these changes including the Pulmonary Rehabilitation Services Accreditation Scheme (PRSAS)

Main duties of the job

To work as part of the Pulmonary Rehabilitation Service providing effective and efficient respiratory therapy to patients over eighteen years of age, who live in the Middlesbrough and Redcar & Cleveland localities, within the framework of clinical governance.

To perform skilled therapeutic assessment of patients for Pulmonary Rehabilitation who present with diverse and complex chronic physical and psychological problems

To deliver Pulmonary Rehabilitation exercise, education programmes, anxiety and fatigue management using evidence based, client centred principles to assess, plan, implement and evaluate interventions.

Reduce acute admissions, reduce exacerbations and increase capacity in primary care by optimising treatment and management of breathlessness, COPD and other respiratory conditions. Improve patient attendance and completion of Pulmonary Rehabilitation.

To work across different venues and locations in the community including patient's own homes

The service aims to enable individuals to maximise their independence and management of long term respiratory conditions.
