Community Heart Failure Nurse Specialist

An exciting opportunity has arisen within Warrington and Halton Cardiac Therapy Services, for a part-time (22.5hrs per week) Band 7 Community Heart Failure Nurse prescriber to join our Team. The hours are to be worked over 3 days per week to include Monday, Tuesday and Wednesdays. 

The post holder will work in both a hospital and community setting within a multidisciplinary team. The post holder is required to support other community health care professionals, to ensure that patients receive optimal care in keeping with evidence-based practice and ensure that patients and carers receive appropriate treatment, and support.

Applicants currently undertaking the Non-Medical Prescribing course may be considered as a band 6 progressing to band 7 once Non-medical Prescribing qualification and competencies are achieved.

The role involves managing a case-load of patients with confirmed heart failure with complex needs. Patients are referred for support post-discharge, from cardiology clinic for optimisation of heart failure medications, or with end-stage heart failure. 

The role will include care of patients  within a community setting, home visits and within a clinic setting at Warrington Hospital. The post holder is required to assess, plan and implement a plan of care for patients within their care as well as support the wider team. The post holder will also be required to optimise heart failure medications as per NICE guidelines and WHH Non-medical prescribing policy.

  • • 2 years post qualification experience in cardiology.

  • • Evidence of Leadership qualifications.
  • • Palliative care experience

Qualifications

  • • Degree/Diploma in nursing RGN.
  • • Current registration with NMC.
  • • Clinical Professional Qualification to a masters level or equivalent.
  • • Nurse Prescriber or commitment to attain within 12 months of commencement.
  • • Evidence of a clinical examination qualification.

  • • Counselling certificate.
  • • Management/ leadership qualification
  • • ENB 998 or equivalent teaching and assessing in clinical practice module.
  • • Clinical diagnostic qualification.

Physical skills
