Integrated CHC Team Nurse Assessor

This is an exciting post within the All Age Continuing Care (AACC) Directorate.   As a Nurse Assessor the post holder will provide specialist nursing assessment and review for adults and older people presenting with long-term care needs who may be eligible for Continuing Health Care, Funded Nursing Care and jointly funded packages of care.   The team is based within our Health and Social Care Hub at Forster House in St Helens although we operate on an agile working basis.

Please note the AACC Team is currently under review and there may be changes made over next few months.

Main duties of the job

The post holder will provide assurance around quality of commissioned care packages and reporting of safeguarding concerns in accordance with agreed interagency reporting procedures.  We are seeking a team player who is proactive, resilient, and driven to succeed with a real commitment to work as part of an integrated team.  the post holder will have experience of partnership working and will be able to demonstrate commitment to highest ethical and professional values.  They will be flexible and open to change in the interests of continuous service improvement.

Detailed job description and main responsibilities

Key responsibilities

1. To perform NHS CHC in line with National Standards and National Tools.
2. To ensure clients receive a fair assessment for CHC funding based on their assessed level of need. This will be done in a variety of settings within an agreed timescale.
3. To perform the assessment, care management and review of clients in the residential setting in collaboration with Social Services.
4. To facilitate the assessment of vulnerable adults/children and to offer support in collaboration with Social Services and other agencies as required.
5. To provide specialist education and training, as required, to staff across the local health economy.

Managerial
1. Manages risk to self and team members in often chaotic, unpredictable situations whilst working with clients either alone or with colleagues.
2. Acts upon referrals from other colleagues/ agencies.
3. Adhere to organisational policies and procedures.
4. To support the Team Manager in the on-going development of the service by identifying areas for improvement as per ICB transformation plans.
5. Liaise with other disciplines to achieve best practice.
6. To participate in appraisal process and personal development.

Qualifications

Essential criteria
  • Registered Mental Health Nurse/ Registered General Nurse
Desirable criteria
  • Evidence of further study

Knowledge & Experience

Essential criteria
  • Experience of working within the NHS.
  • Knowledge of the revised National Framework for NHS Continuing Healthcare Funding.
Desirable criteria
  • Experience in supervision/ line management.

Other

Essential criteria
  • Ability to understand the continuing healthcare and funded nursing care criteria and the ability to apply this to everyday practice
  • Ability to communicate to Multi Agencies
  • Ability to prioritise and organise workload
  • Ability to manage conflict with people with challenging behaviour.
  • Commitment to quality.
  • High workload capacity and capable of sustained effort over a long period – able to work under pressure to tight deadlines.
